Differences in Structure and Function of Optical Nerve in Multiple Sclerosis and Neuromyelitis Optica Spectrum Disorders and Its Relation to Degree of Disability, Duration of Disease, and Number of Relapse

Background. MS and NMOSD is a disease that results in demyelinization of the central nervous system. This study aims to determine the differences in structure and function of the optic nerve in MS and NMOSD patients and to see its relationship with EDSS, duration of disease, and number of relapse. Method. 30 MS eyes and 15 NMOSD eyes were assessed by using OCT, fundus photography, visual acuity and contrast sensitivity examination, and P100 latency. Results. NMOSD group has lower GCL-IPL and RNFL thickness and also lower visual acuity than MS. In this group there was also a positive correlation between EDSS value with visual acuity and negative correlation with mean GCL-IPL and RNFL thickness. The positive correlation between disease duration and visual acuity and negative correlation with GCL-IPL layer thickness was seen in the NMOSD group. While in MS, there was a negative correlation between duration of disease with contrast sensitivity and mean GCL-IPL and RNFL thickness. The number of relapse were negatively correlated with mean RNFL thickness in the NMOSD group. In the MS group, the number of relapse was not correlated with the structural and functional parameters of the optic nerve. Conclusion. NMOSD group has thinner GCL-IPL and RNFL thickness. The optic nerve function in NMOSD is also inferior to MS. The degree of disability and duration of disease correlates with the structural and functional parameters of the optic nerve in MS and NMOSD patients. While the correlation with the number of relapse is only found in the NMOSD group.
