Background: Management for male infertility can be difficult for some cases. Surgical intervention has long been thought as the last resort to help married couples to conceive. The current guideline recommends testicular sperm extraction with microsurgery technique (microTESE) in severe cases of male infertility. However, the success rate still varies. Thus, a new strategy was needed to further increase the sperm retrieval success rate. Case Presentation: A 39-year-old male with a history of failed sperm extraction, non-obstructive azoospermia (NOA) and Y-chromosomal microdeletion came to the fertility center to undergo sperm retrieval. Fine needle aspiration (FNA) Mapping was performed prior to microTESE to increase the accuracy of sperm retrieval. After further examination with laser assisted immotile sperm selection (LAISS), five spermatozoa were found. Purpose: This study aimed to provide the method of preparation, characterization of curcumin-loaded chitosan-sodium tripolyphosphate (NaTPP) nanoparticle, and evaluate its pharmacokinetic profiles. Methods: Curcumin-loaded chitosan-NaTPP nanoparticles were synthesized using ionic gelation methods. Curcumin was dissolved using surfactants and cosurfactants. Chitosan polymer was then mixed in the curcumin solution and dripped with NaTPP solution until nanoparticle formation. The mucoadhesive study was evaluated by measuring the fluorescence of curcumin within the prepared nanoparticles. The pharmacokinetic profiles of curcumin particles and nanoparticles were then assessed in rats by administering a single oral dose of 100 mg/kg BW. Blood samples were taken from nine predetermined time points, and curcumin plasma concentrations were then analyzed using UPLC-MS/MS. Results: The particle size of the curcumin nanoparticles obtained were 11.5 nm. Entrapment efficiency (EE) of curcumin nanoparticles were exceeding 99.97%, and drug loading capacity (DLC) was 11.34%. The mucoadhesive properties of the nanoparticles were superior to that of curcumin particles. Pharmacokinetic evaluation in rats revealed that curcumin nanoparticles resulted in an increase of area under the curve (AUC), maximum concentration (Cmax), earlier time to reach maximum concentration (Tmax), and lower clearance (CL). Conclusion: Curcumin-loaded chitosan-NaTPP nanoparticles is an effective formulation to improve curcumin plasma concentrations. Background: Cardiac index (CI; cardiac output indexed to body surface area) is routinely measured during kidney transplant surgery. Bioimpedance cardiometry is a transthoracic impedance as the non-invasive alternative for hemodynamic monitoring, using semi-invasive uncalibrated pulse wave or contour (UPC) analysis. Objectives: We performed a cross-sectional observational study on 50 kidney transplant patients to compare the CI measurement agreement, concordance rate, and trending ability between bioimpedance and UPC analysis. Methods: For each patient, CI was measured by bioimpedance analysis (ICON™) and UPC analysis (EV1000™) devices at three time points: after induction, during incision, and at reperfusion. The device measurement accuracy was assessed by the bias value, limit of agreement (LoA), and percentage error (PE) using Bland-Altman analyses. Trending ability was assessed by angular bias and polar concordance through four-quadrant and polar plot analyses. Results: From each time point and pooled measurement, the correlation coefficients were 0.267, 0.327, 0.321, and 0.348. BlandAltman analyses showed mean bias values of 1.18, 1.06, 1.48, and 1.30, LoA of-1.35 to 3.72,-1.39 to 3.51,-1.07 to 4.04, and-1.17 to 3.78, and PE of 82.21, 78.50, 68.74, and 74.58%, respectively. Polar plot analyses revealed angular bias values of-10.37º,-15.01º,-18.68º, and-12.62º, with radial LoA of 89.79º, 85.86º, 83.38º, and 87.82º, respectively. The four-quadrant plot concordance rates were 70.77, 67.35, 65.90, and 69.79%. These analyses showed poor agreement, weak concordance, and low trending ability of bioimpedance cardiometry to UPC analysis. Conclusions: Bioimpedance and UPC analysis for CI measurements were not interchangeable in patients undergoing kidney transplant surgery. Cardiac index monitoring using bioimpedance cardiometry during kidney transplantation should be interpreted cautiously because it showed poor reliability due to low accuracy, precision, and trending ability for CI measurement. © 2021, Author(s). |

Background: Malaria is the second most life-threatening infectious disease in Indonesia, causing approximately 1-3 million deaths annually. Histopathologic studies assessing CD 68 and CD 34 protein expression in placental malaria and its association with maternal anemia are essential to determine the prognosis of malaria in pregnancy. Materials and Methods: This cross-sectional study was carried out in 2017. Thirty biopsy samples of human placental tissue were obtained from Timika and Sumba, and ten normal biopsy samples were taken from the Pathological Anatomy Department of Cipto Mangunkusumo General Hospital as comparisons. CD 34 and CD 68 protein expressions were determined using immunohistochemistry, and the resulting data were analyzed using SPSS. Results: Average hemoglobin (Hb) level was 9.5 mg/dL, 11.5 mg/dL, and 9.9 mg/dL in acute infection, chronic infection, and latent infection, respectively. A positive correlation was found between CD 68 protein expression and maternal Hb level. No correlation was found between CD34 expression and maternal anemia. Conclusions: CD 68 expression in placental tissue biopsy from Timika and Sumba residents with placental malaria was shown to be positively correlated with maternal anemia. Immunohistochemical examination of CD 68 may play a role in the early diagnosis of malaria. © 2021 Tropical Parasitology | Published by Wolters Kluwer - Medknow. |

Introduction: Mortality in pediatric septic shock remains very high. Vasoactive-inotropic score (VIS) is widely used to predict prognosis in patients with heart disease. It is a simple method that was initially used as a predictor of morbidity and mortality in postoperative patients with congenital heart diseases. Previous reports showed that high VIS score was associated with high mortality in pediatric sepsis. However, its discriminative value remains unclear. We aim to explore the discriminative value of VIS in predicting mortality in pediatric septic shock patients. Methods: We conducted a retrospective cohort study on medical records of septic shock patients who received care in the pediatric intensive care unit (PICU). We screened medical records of pediatric patients which were diagnosed with septic shock and admitted to the PICU and received vasoactive/inotropic score for more than 8 h. Other supporting examination results were recorded, such as organ function evaluation for calculation of Pediatric Logistic Organ Dysfunction-2 (PELOD-2) score. The outcome of patients was recorded. The receiver operating curve was constructed to calculate the area under the curve (AUC), sensitivity, and specificity of each cutoff point. Results: We obtained the optimum cutoff point of VIS > 11 with 78.87% sensitivity and 72.22% specificity. AUC positive was 0.779 (P < 0.001); predictive value and negative predictive value were 91.80% and 46.43%, respectively. Context: Adiponectin is an adipokine that is secreted by adipocytes and acts to prevent obesity and obesity-related disorders. The high ratio of omega-6 to omega-3 fatty acids in modern dietary habits in working-Age populations, urban areas in particular, is known to play a role in adiponectin secretion. However, previous studies showed inconclusive results. Aim: The study aims to determine the association of the dietary omega-6 to omega-3 fatty acids ratio with adiponectin levels in office workers in Jakarta, Indonesia. Settings and Design: This cross-sectional study included 55 office workers in East Jakarta, Indonesia. Methods: Data were collected via questionnaire, 3-day food record, anthropometric measurement, and serum collection. Statistical analysis used: Independent t test was applied to assess the relationship between subjects characteristics and dietary intake with high molecular weight adiponectin levels. Association of dietary omega-6/omega-3 fatty acids ratio and adiponectin were evaluated using Pearson correlation test (P < 0.05). Results: There is no statistically significant difference in adiponectin levels based on waist circumference category, as well as categories of physical activity, household income levels, total energy, and total fat intake. Conversely, in female subjects (P = 0.000), subjects with normal body mass index (P = 0.000), higher education levels (P = 0.031), and nonsmoking subjects (P = 0.019), significantly higher adiponectin levels are obtained. The ratio of dietary omega-6/omega-3 fatty acids is negatively correlated with adiponectin (P = 0.004, r =-0.383). Conclusions: The decrease in the ratio of omega-6/omega-3 fatty acids is inversely related to higher level of adiponectin, indicating that dietary changes may potentially play a role in prevention strategies for obesity and obesity-related diseases. © 2021 Wolters Kluwer Medknow Publications. Postmenopausal hypoestrogenism is associated with skin aging, for which phytoestrogen derived from the seeds of Trigonella foenum graecum (fenugreek) is expected to be an alternative solution to reduce wrinkles and increase the thickness of postmenopausal women’s skin. This study was a randomized, double-blind, controlled 12-weeks trial conducted at a general hospital in Jakarta, Indonesia, from January to November 2019. Subjects were 50 postmenopausal women divided into two groups: the intervention group was given 5% fenugreek cream and base cream was given to the placebo group. The results of the independent t-test showed that both groups were able to achieve statistically significant improvement in wrinkle scores on the forehead, crow’s feet, and nasolabial folds which was assessed by photography scoring based on Bazin’s skin aging atlas for Asian skin volume 2 but there was no significant disparity between both groups at all time points. Dermal thickness was assessed by high-resolution ultrasound GE LOGIQ E9 (18 MHz), which showed similar results for both groups with significant improvements in the 8th week compared to baseline and a significant decrease by the 12th week. No significant differences were observed in the skin wrinkle score and skin thickness after the application of fenugreek cream compared to placebo.We suspect that a concentration of 5% was not adequate for the expected antiskin aging effects. Further studies are necessary to determine a more appropriate fenugreek concentration to permit clinical use as an antiskin aging therapy in postmenopausal women. © 2021 Shannaz Nadia Yusharyahya et al. A previous study showed that alpha-mangostin (AM) showed benefit when given to sorafenib (SOR)-surviving cells. However, the mechanism was not fully understood. The present study aimed to understand the effect of AM on SOR-surviving cells and its agent concerning drug transporters. SOR-surviving cells were treated with SOR 10 μM. Surviving cells were divided into four groups of treatment, namely, vehicle only dimethyl sulfoxide (DMSO), SOR 10 μM, AM 20 μM, or combination of SOR 10 μM-AM 20 μM. As controls, HepG2 naïve cells were treated with DMSO only or AM 20 μM. Cell viability was counted using trypan blue exclusion assay. Simultaneously, the mRNA expressions of P-glycoprotein (P-gp), ABCG2, MRP2, MRP3, OCT1, and OATP1B3 drug transporters were examined with quantitative reverse transcriptase-polymerase chain reaction. Decreased mRNA expression of P-gp was found in SOR-surviving cells treated with SOR. In contrast, AM alone or SOR's combination caused a significant increase in both efflux and influx transporters, no difference in fold increase of all transporters evaluated in AM versus SOR-AM combinations. Breast cancer is one of the most diagnosed cancers in women; the number of cases continues to rise. The high prevalence and increased incidence need more attention in developing effective therapy. Current passive therapy has several drawbacks that have not yet been resolved. Thus, an alternative and preventive therapy for cancer is needed by utilizing vaccines. Immunoinformatics approach is one of the promising methods predicting epitopes in vaccine research. This approach could accelerate the initial study process of vaccine development and reduce research costs. Epitope conservancy and vaccine coverage are important parameters in vaccine research due to addressing the variability and diversity of cancer genomics. This study will be carried out on the multiepitope characterization of potential T cells against the protein mechanism in breast cancer. Proteins used in this study are Mucin-4, Phosphatase And Tensin Homolog, and Receptor tyrosine-protein kinase erbB-2. CTL epitopes, antigenicity, immunogenicity, allergenicity, and toxicity were predicted for the peptide vaccine. Immunoinformatics analysis generates a multiepitope sequence consisting of seven epitopes: DPVALVAPF, SVAYRLGTL, SQINTLNTL, RFRELVSEF, VTSANIQEF, RPRFRELVS, and MYFEFPQPL by AAY linker. The docking and molecular dynamics analyses were conducted to confirm the interactions between the multiepitope vaccine molecule and TLR-4-MD. Tyrosine kinase inhibitors (TKIs; e.g., erlotinib, gefitinib, and afatinib) are the first-line therapy for non-small-cell lung cancer (NSCLC) patients with epidermal growth factor receptor (EGFR) (+) common mutation. The study’s objective was to analyze the cost-effectiveness of erlotinib, gefitinib, and afatinib in NSCLC patients. The subjects of the study were NSCLC patients with EGFR (+) mutation receiving either erlotinib, gefitinib, or afatinib from January 2017 to December 2019. The exclusion criteria were patients receiving the respective therapy for less than 2 months and patients unable to complete the treatment until after December 2019. The parameter of treatment effectiveness was progression-free survival (PFS), which was measured as the time from initiation of the therapy until disease progression occurred or a patient became deceased. Direct medical costs, from the hospital perspective, were calculated during the treatment. A nonparametric Kruskal-Wallis test was conducted to compare the median PFS and direct medical cost between the three treatment groups. The median PFS of patients receiving erlotinib, gefitinib, and afatinib was 8 months, 12 months, and 5 months, respectively. There were significant differences in the monthly direct medical costs between the study groups: erlotinib (IDR 13,545,116), gefitinib (IDR 14,727,887), and afatinib (IDR 12,146,834). The cost-effectiveness ratio of the study groups was as follows: erlotinib IDR 1,693,139.50/months; gefitinib IDR 1,227,323.92/months; and afatinib IDR 2,429,366.80/months.
