Morphologic feature of skin and hair abnormalities becomes more visible under this tool compared with naked eye. It helps to diagnose hair disorders faster and can be used for follow up treatment in patients. The purpose of this study is to compare superiority between clinical diagnosis alone and with addition of dermoscopy tool in the assessment of hair disease. Methods: Research was conducted in October 2018 in a boarding school in West Java, Indonesia. This was a cross-sectional descriptive study. Statistical analysis was done using SPSS Statistics 20.0. Data were collected from 127 subjects through anamnesis, general physical examination and dermatological findings, pull test, dermoscopy, and photographs of scalp and hair lesions. Results: The highest incidence of hair disorder in this study found by clinical examination and dermoscopy was pediculosis capitis, with a total of 86 subjects (67.7%). From the comparison between clinical examination and dermoscopy, we found that dermoscopy was superior for diagnosis. As much as 43 cases of hair disorders which were not detected on clinical examination screening were found by dermoscopy. Out of 127 children, there were 49 subjects with hair disorders found in clinical examination and 92 subjects found by dermoscopy. Conclusion: Dermoscopy as a non-invasive tool which plays an important role in determining the diagnosis of hair disorder. © 2021 Pakistan Association of Dermatologists. All rights reserved. This study aimed to determine the quantity of antibiotic use based on the defined daily dose (DDD) per 100 bed days and the quality of antibiotic use based on the percentage of concordance with The American Society of Health-System Pharmacists (ASHP) therapeutic guidelines (i.e., route of administration, time of administration, selection, dose, and duration). This includes the profiles of surgical site infection (SSI) in surgical patients from January through June 2019. The study subjects were 487 surgical patients from two hospitals who met the inclusion criteria. There were 322 patients out of 487 patients who had a clean surgical procedure. Ceftriaxone (J01DD04) was the most used antibiotic, with a total DDD/100 bed days value in hospitals A and B, respectively: pre-surgery: 14.71, 77.65, during surgery: 22.57, 87.31, and postsurgery: 38.34, 93.65. In addition, 35% of antibiotics were given more than 120 min before incision. The lowest concordance to ASHP therapeutic guideline in hospital A (17.6%) and B (1.9%) was antibiotic selection. Two patients experienced SSI with bacterial growths of Proteus spp., Staphylococcus aureus, Staphylococcus epidermidis, and Escherichia coli. The usage of prophylactic antibiotics for surgical procedures was high and varied between hospitals. Hospital B had significantly lower concordance to antibiotic therapeutic guidelines, resulting to a higher infection rate, compared with hospital A. ASHP adherence components were then further investigated, after which antibiotic dosing interval and injection time was found to be a significant predictor for positive bacterial growth based on logit–logistic regression. Further interventions and strategies to implement antibiotic stewardship is needed to improve antibiotic prescriptions and their use. © 2021 by the authors. Licensee MDPI, Basel, Switzerland. Vaccines that target conserved regions of SARS-CoV-2 and stimulate protective T-cell responses are important for reducing symptoms and limiting the infection. Seven cytotoxic (CTL) and five helper T-cells (HTL) epitopes from ORF1ab were identified using NetCTLpan and NetMHCIIpan algorithms, respectively. These epitopes were generated from ORF1ab regions that are evolutionary stable as reflected by zero Shannon’s entropy and are presented by 56 human leukocyte antigen (HLA) Class I and 22 HLA Class II, ensuring good coverage for the Indonesian and world population. Having fulfilled other criteria such as immunogenicity, IFNγ inducing ability, and non-homology to human and microbiome peptides, the epitopes were assembled into a vaccine construct (VC) together with β-defensin as adjuvant and appropriate linkers. The VC was shown to have good physicochemical characteristics and capability of inducing CTL as well as HTL responses, which stem from the engagement of the vaccine with toll-like receptor 4 (TLR4) as revealed by docking simulations. The most promiscuous peptide899WSMATYYLF907 was shown via docking simulation to interact well with HLA-A*24:07, the most predominant allele in Indonesia. This study aims at assessing several scenarios of delivering the vaccine to people in Indonesia. We develop a model for several scenarios of delivering vaccines: without vaccination, fair distribution, and targeted distribution to five and eight districts with the highest COVID-19 incidence in West Java, one of the most COVID-19-affected regions in Indonesia. We calculate the needs of vaccines and healthcare staff for the program, then simulate the model for the initial 4-month and one-year scenarios. A one-year vaccination program would require 232,000 inoculations per day by 4833 vaccinators. Targeted vaccine allocation based on the burden of COVID-19 cases could benefit the COVID-19 vaccination program by lowering at least 5000 active cases. The benefits would increase by improving the number of vaccines and healthcare staff. Amidst lacking available vaccines, targeted vaccine allocation based on the burden of COVID-19 cases could increase the benefit of the COVID-19 vaccination program but still requires progressive efforts to improve healthcare capacity and vaccine availability for optimal protection for people. © 2021 by the authors. Exposure to this hormone resulted in insulin receptor phosphorylation (pTyr972), activation of AKT (pSer473) and the stimulation of sperm motility. Intriguingly, the male germ line is also shown to be capable of generating insulin, possessing the relevant mRNA transcript and expressing strong immunocytochemical signals for both insulin and C-peptide. Insulin could be released from the spermatozoa by sonication in a concentration-dependent manner but was not secreted in response to glucose, fructose or stimulation with progesterone. However, insulin release could be induced by factors present in human uterine lavages. Furthermore, the endometrium was also shown to possess the machinery for insulin production and action (mRNA, insulin, C-peptide, proprotein convertase and insulin receptor), releasing insulin into the uterine lumen prior to ovulation. These studies emphasize the fundamental importance of extra-pancreatic insulin in regulating the reproductive process, particularly in the support of spermatozoa on their perilous voyage to the site of fertilization. © 2021 Elsevier B.V. The hepatic vein ostial stenosis and short segment thrombosis are common in some parts of Asia-Pacific region, while membranous obstruction of the vena cava is common in some and complete thrombosis of hepatic veins in others. Prevalence of myeloproliferative neoplasms and other thrombophilic disorders in BCS varies from region to region and with different sites of obstruction. This heterogeneity also raises several issues and dilemmas in evaluation and approach to management of a patient with BCS. The opportunity to recanalize hepatic vein in patients with hepatic vein ostial stenosis or inferior vena cava stenting or pasty among those membranous obstruction of the vena cava is a unique opportunity in the Asia–Pacific region to restore hepatic outflow closely mimicking physiology. In order to address these issues arising out of the diversity as well as the unique features in the region, the Asia Pacific Association for Study of Liver has formulated these guidelines for clinicians. © 2021, Asian Pacific Association for the Study of the Liver. Multimodal analgesia is one of the modalities of overcoming postoperative pain. The use of a combination of paracetamol and ibuprofen is expected to reduce the total morphine requirement after TKA. OBJECTIVES: The use of a combination of paracetamol and ibuprofen to reduce morphine requirement after TKA, to provide adequate pain management and early ambulation. STUDY DESIGN: Patients scheduled for total knee arthroplasty who met the requirements for inclusion criteria were consented and randomized using randomizer.org in a 1:1:1 ratio to receive either combination paracetamol iv and ibuprofen iv (Group II), paracetamol iv only (Group II), or ibuprofen iv only (III). SETTING: Thirty-six patients aged 63-68 years who underwent TKA were included in this study. METHODS: All patients were divided into 3 groups. Group I received paracetamol 1 g and ibuprofen 800 mg, group II received 1 g paracetamol iv and 100 mL normal saline, group III received 800 mg ibuprofen iv and 100 mL normal saline, 10 minutes before the end of surgery and every 6 hours up to 24 hours. Total morphine consumption, pain score (resting, walking, knee flexion), and 2 minute-length walking tests were measured in hour 24 postoperative. Data were analyzed with SPSS 16.0. RESULTS: Median of total morphine consumption between 3 groups respectively was 7.5 (3.0-36.0) mg vs 15.0 (4.5-28.5) mg vs 9.0 (0.0-24.0) mg with no difference (P = 0.391). Mean of pain score at walking phase respectively was 4.8 ± 0.5 vs 7.3 ± 1.2 vs 5.6 ± 0.5 (hour 24, P < 0.01). Medians of 2-Minute Walking Test respectively were 6.0 (2-16) meters vs 0.0 (0-4) meters vs 0.0 (0-4) meters (hour 24, P < 0.01). LIMITATIONS: The total morphine requirement measured in this study illustrates the consumption of morphine in resting phase. CONCLUSION: The combination of paracetamol and ibuprofen is better in reducing the total morphine requirement after TKA when compared with the administration of paracetamol injection alone or ibuprofen injection alone. Methods: A systematic review was performed using the approach recommended by the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines for meta-analyses of interventional studies. The comprehensive search was conducted using the following database: MEDLINE, EMBASE, and the Cochrane Library. Patient characteristic, intervention approaches, embolic agents, and results at mid and long term follow up were studied. Results: A total of 6 studies corresponding to a total of 141 patients fulfilled the inclusion criteria with a mean age of 73–78.6 years and a mean duration of follow up varying from 25 to 42 months. There were different techniques for embolization used (translumbar, transarterial, and transcaval approach) with various types of embolic agents. In all studies, the indication for embolization of the type II endoleaks was sac enlargement of more than 5 mm. A wide range of technical success rate was reported regardless of the intervention strategy being used (17,6%–100%). The overall technical success rate of all studies was 62%. Conclusion: This systematic review shows that there is a wide variety of techniques to exclude a persistent type II endoleak. Different kinds of embolic agents have be used. Due to a lack of peer reviewed data on longterm follow-up, it was not possible to come to recommendations what treatment would be the best for a durable exclusion of a persistent type II endoleak after an initially successful EVAR. Meanwhile, going to hit the ball requires excellent attention for an individual student. Exhibiting gamma waves further influence arrangements of hitting performance. The objectives of this study were first, to determine the negative functional correlation between gamma brain waves and hitting skills, then second to know the positive functional correlation between attention and hitting skills and third to determine the effect of attention on increasing hitting skills. The method used in this research is an experimental method with a one-group pretest-posttest design research design. The sampling technique in this study was using a saturated sampling technique. This investigation amounted to 20 subjects, in the calculation of the Pearson product-moment correlation test using SPSS v.23. The first results collected were p-value 0.026 with an r square value of 0.25, so there is a significant negative functional correlation between gamma brain waves and hitting skills in softball learning of 25%. The second issue obtained p-value 0.017 with an r square value of 0.28, so there is a significant positive functional correlation between attention and hitting skills in softball learning by 28%. Furthermore, thirdly, the results obtained p-value 0.0001 between pre-test and post-test, with a significant increase in skills of 70.17%, so there is a significant influence between attention and hitting skills. © 2021 by authors, all rights reserved.
