Wharton's jelly mesenchymal stem cells (WJ-MSCs) is proposed to be the best source of mesenchymal stem cells (MSCs). It has better self-renewal and differentiation capacity compare to adult stem cells such as bone marrow- derived stem cells (BM-MSCs). However, their characteristics in different passages do not yet well described. This research aims to isolate and characterize WJ-MSCs with regard to their proliferation capacity, immunophenotypes, and differentiation capacity. The WJ-MSCs were isolated by the explant method. The cultured WJ-MSCs at P5 and P8 were then characterized using flow cytometric analysis with regard to its surface marker, which are CD44, CD90, CD105, CD73, and negative lineages. The proliferation capacity of WJ-MSCs was analyzed with the population doubling time method and the characterized cells were differentiated into three different lineages. The result revealed that the isolated cells highly express MSCs surface marker with >98% for all surface markers and 0.00% for negative lineages. Isolated WJ-MSCs also exhibited a high proliferation capacity with a range of 1.06 to 1.59 days. Both passages revealed excellent differentiation capacity into chondrocytes, adipocytes, and osteocytes. © 2021 Author(s). |

Background: Childhood pneumonia is the leading cause of death. Family caregivers may have a poor understanding of pneumonia, especially related to prevention and treatment. It is crucial to understand country-specific knowledge and healthcare-seeking behaviors of caregivers of children with pneumonia before planning programmatic responses, particularly in an urban community where the culture-social economic status is diverse. Objective: This study aimed to describe the knowledge and healthcare-seeking behavior of family caregivers of children with pneumonia in Indonesia's urban community. Methods: A descriptive qualitative study was conducted at a public hospital in Jakarta, Indonesia, from 10 December 2019 to 28 January 2020. Ten family caregivers of children with pneumonia were included, and a semi-structured interview was conducted for data collection. The audio recording was transcribed verbatim, and the data were analyzed using content analysis. Results: The emerged data indicated low knowledge of the family caregivers, especially mothers, which considered pneumonia a common cold only, and they failed to understand the risk of its transmission. Using traditional medicines was the first stage of healthcare-seeking behavior. If the symptoms worsened, the caregivers brought their children to the nearest health center. If there was no change in the symptoms, they headed to the hospital. The health center's location, condition, services, and staff attitude were considered factors to choose the services. Conclusion: Living in an urban area does not guarantee the family caregivers have better knowledge and good healthcare-seeking behavior. Therefore, nurses should provide comprehensive education about pneumonia, its symptoms, and treatment management to improve family care and prevent pneumonia-related deaths. Integrated management of children with illness is recommended to help the family caregivers of children with pneumonia receive treatment as quickly as possible. © The Author(s) 2021 |

The COVID-19 pandemic is now catching the country by surprise. The disease has had a huge influence worldwide, and the nation has been put in lockdown by this health crisis. The psychological health, physical health and economic condition of individuals are adversely impacted by quarantine and lockdown. As mobility is reduced, there will be a dramatic shift in the population's lifestyle, where the people's physical activity level will decrease. Low levels of physical exercise can lead to a large number of obesity cases. Therefore, steps for obesity prevention and management need to be taken. One approach is to administer nutrition with accuracy through precision nutrition. Therefore, this research aims to review articles on precision nutrition for the treatment and control of obesity towards a restricted movement order during the COVID-19 health crisis. The literature search was conducted from May 2020 until January 2021 using Scopus and Science Direct as the main databases. The articles included follow the inclusion criteria published in English language, including nutrition intervention for prevention and management of obesity, focus on health crisis, and no restriction of time and study design. Various studies show that precision nutrition has a potential benefit in preventing or controlling obesity prevalence due to restricted movement order or quarantine upon health crisis. © 2021 Lahore Medical And Dental College. Community-based quarantine can slow down the pandemic growth while adversely affecting population-wide psychological well-being. Affected psychological well-being could potentially influence population compliance in following stipulated community quarantine procedures. Aim: The aim was to quantify psychological distress among Greater Jakarta area residents during the community containment period. Objectives: The objective was to measure depression, anxiety, and stress levels using the Indonesian version of the DASS-21. Demographic data on sex, education strata, and working/productive-age group were also collected. Methods: This cross-sectional observational analytic study employed an online questionnaire involving participants acquired through snowball sampling. The questionnaire comprises two parts: demographic data and psychological distress indicators. Linear regression evaluated psychological distress as a response variable. Results: Among 1,205 women and 824 men, our findings suggested male sex, age in the range of 15-24 years, and having a bachelor's degree or professional qualification have a strong association with psychological distress. Conclusion: By addressing the population at risk, policymakers can identify better countermeasures for preventing psychological distress. © 2021 The Authors |

Introduction: Spondylitis tuberculosis can cause changes in spinopelvic parameters including pelvic incidence, pelvic tilt, and sacral slope due to biomechanical changes of the spine. Posterior instrumentation is one of the modality for the treatment of spondylitis tuberculosis. However, in Indonesia, clinical and radiological outcomes after posterior instrumentation in tuberculosis of lumbar vertebrae are still rare. This study aims to investigate the clinical and radiological outcomes of patients with spondylitis tuberculosis of the lumbar vertebrae after posterior instrumentation. Method: This study was a cross-sectional study in patients with spondylitis tuberculosis of the lumbar vertebrae who underwent posterior instrumentation in Cipto Mangunsukumo and Fatmawati Hospital. Subjects were collected through consecutive sampling. 23 subjects were collected and analyzed. Clinical and radiological outcomes before and after posterior instrumentation were compared. The clinical outcome included the Visual Analog Scale (VAS) and Oswestry Disability Index (ODI). The radiological outcome included sacral slope, pelvic tilt, pelvic incidence, and lumbar lordosis. Results: The median age of the subjects was 31 (9–57) years with a female-majority (60,9%). The median of the total vertebral infected was 2 (1–4). Median of VAS score before surgery, 6 months after surgery, and 12 months after surgery were 9 (4–10), 4 (1–7), dan 2 (1–6) (p < 0,001) consecutively. Median of ODI score before surgery, 6 months after surgery, and 12 months after surgery were 70 (40–86), 34 (10–74), dan 12 (2–74) (p < 0,001) consecutively. There was no significant difference in spinopelvic parameters before and after the surgery. The difference of ODI score before and after the surgery inversely correlated with the difference of lumbar lordotic and sacral slope. Conclusion: Posterior instrumentation could improve clinical outcomes in patients with spondylitis tuberculosis of the lumbar. Change of lumbar lordotic and sacral slope after posterior instrumentation led to an improvement of quality of life marked by the reduction of the ODI score. © 2021 The Authors |

Hemifacial spasm, Microvascular decompression, Intraoperative monitoring, Lateral spread response, Abnormal muscle response. © 2021 The AuthorsBackground: Microvascular decompression has been established as a primary treatment for hemifacial spasm. Intraoperative monitoring is used during the surgery to guide neurosurgeons to determine whether the decompression of facial nerve from the vessel is sufficient. We performed a systematic review to assess the role of lateral spread response (LSR) monitoring in predicting hemifacial spasm outcomes after microvascular decompression. Method: A systematic search of PubMed, ScienceDirect, Cochrane, and Google Scholar was conducted. We included studies that performed microvascular decompression surgery with intraoperative monitoring analyzing the correlation between lateral spread response and spasm relief. A critical appraisal was conducted for selected studies. Result: Twenty-two studies comprising 6404 cases of hemifacial spasm, which underwent microvascular decompression surgery with intraoperative monitoring, were included. Of 15 articles that assessed symptoms shortly after surgery, 12 studies showed a significant correlation between lateral spread response resolution and disappearance of spasm. Four of six studies that evaluated the outcome at 3-month follow-up showed significant relationship between LSR and outcome, so did five of six articles that assessed spasm relief at 6-month follow-up. As much as 62.5% of studies (10 of 16) showed the result at long-term follow-up (≥1-year) was not significant. Conclusion: Intraoperative monitoring during microvascular decompression surgery can be a useful tool to predict hemifacial spasm resolution. Though long-term outcomes of patients with LSR relief and persistence are similar, resolution of symptoms shortly after surgery will provide comfort to patients thereby improving their quality of life. © 2021 The Authors |

In Indonesia, dengue has become one of the hyperendemic diseases. Dengue consists of three clinical phases—febrile phase, critical phase, and recovery phase. Many patients have died in the critical phase due to the lack of proper and timely treatment. Therefore, we developed models that can predict the severity level of dengue based on the laboratory test results of the corresponding patients using Artificial Neural Network (ANN) and Discriminant Analysis (DA). In developing the models, we used a very small dataset. It is shown that ANN models developed using logistic and hyperbolic tangent activation function with 70% training data yielded the highest accuracy (90.91%), sensitivity (91.11%), and specificity (95.51%). This is the proposed model in this research. The proposed model will be able to help physicians in predicting the severity level of dengue patients before entering the critical phase. Furthermore, it will ease physicians in treating dengue patients early, so fatal cases or deaths can be avoided. © 2021 by the authors. Licensee MDPI, Basel, Switzerland. |

Coronavirus disease 2019 (COVID-19) is a novel disorder caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Although it mainly affects the respiratory system, the pancreas could also become the virus' target. The issue regarding pancreatic involvement in COVID-19 has been raised by several researchers. They found increased serum amylase and/or lipase in COVID-19 patients, which suggested pancreatic injury. We aimed to critically review the evidence to provide insights and to answer the very question of the possibility of pancreatic injury. Current evidence shows that increased amylase and/or lipase is not necessarily a pancreatic injury in COVID-19 patients. Those increased enzymes might also be found in other clinical conditions. © 2020 The Authors. Objective: Increased levels of inflammatory factors in newborns are often associated with lower maternal vitamin D levels. This study aimed to find out the relationship between maternal and umbilical cord vitamin D serum levels on umbilical cord Interleukin-6 (IL-6) and serum C-Reactive Protein (CRP) levels in premature infants. Methods: The study was an observational analytic, cross-sectional design in mothers who underwent preterm birth at 28-34 weeks' gestation due to premature rupture of membranes (PROM) and their infants at Dr. Cipto Mangunkusumo General Hospital (RSCM), Jakarta and Persahabatan General Hospital, Jakarta, from January 2017 to August 2018. Levels of serum vitamin D of the maternal and umbilical cord, umbilical cord IL-6 and serum CRP in premature infants were recorded. Vitamin D level was divided into deficiency (<10 ng/mL), insufficiency (10-29 ng/mL), and normal (≥30 ng/mL) groups. The relationship of vitamin D levels with IL-6 and CRP was carried out using Kruskal Wallis test. Results: A total of 70 subjects met the research criteria. Umbilical cord IL-6 and serum CRP levels in premature infants of vitamin D deficient mothers were higher (20.31 pg/mL and 0.50 mg/L) compared to insufficient (3.34 pg/mL and 0.45 mg/L) and normal mothers (3.29 pg/mL and 0.30 mg/L), although not statistically significant (IL-6 p = 0.665, CRP p = 0.89). Referring to the umbilical cord blood vitamin D levels, the results were different and not as expected, in which the umbilical cord IL-6 and serum CRP levels of preterm infants in the deficiency (3.76 pg/mL and 0.35 mg/L) and insufficiency (3.37 pg/mL and 0.40 mg/L) groups were lower (IL-6) and not different (CRP) than the normal group (9.41 pg/mL and 0.40 mg/L). Conclusions: There were atendency for an increase in umbbilical cord IL-6 and serum CRP level in premature infants of Vitamin D deficient mother although these were not statistically significant. Based on the levels of vitamin D umbilical cord blood, the CRP levels in the serum of premature infants were not different, while the IL-6 levels in the deficiency and insufficiency group were lower than in the normal group. © Creative Commons Atribuição-Não Comercial 4.0 Internacional |

Objective: To investigate the effect of lifestyle modification on endometrial receptivity of obese women with polycystic ovary syndrome using ultrasonography. Methods: This observational study was conducted at Dr. Cipto Mangunkusumo General Hospital Jakarta from August 2019 to May 2020. A total of 14 subjects were participated in this study. The subjects were advised to get nutrition counseling by nutritionist and then followed up the endometrial profiles for 6 months by ultrasonography. Results: A total 19 subjects were received nutrition counseling by Clinical nutritionist, but then, only 14 subjects were evaluated the endometrial study by Ultrasonography. There were several significant result between before and after treatment some subjects such as calories, body weight, body mass index, waist circumference (p<0.05) and type of the endometrial vascular zone, endometrium volume, and vascular flow index by ultrasonography (p<0.05). In this study, no significant results have been found on the correlation between dietary changes and changes in endometrial receptivity profiles. Conclusions: No significant correlation was observed between changes in anthropometrics and daily calorie intake with changes in endometrial vascular zones. © Indonesian Journal of Obstetrics and Gynecology 2021. |
