BACKGROUND: Melasma has been suspected to be linked with levels of thyroid hormone. There is no study that explains the association between thyroid hormone levels with melasma severity. AIM: This study aims to find the discrepancies in the levels of thyroid hormone in varying severity of melasma using two different measurement techniques. METHODS: Subjects were chosen consecutively from the dermatology clinic at RSUPN Dr. Cipto Mangunkusumo hospital. Forty-eight patients participated in this study were categorized into mild melasma and moderate-severe melasma based on modified melasma area and severity index (mMASI) and Janus II measurement. RESULTS: Statistically, mMASI measurement showed no significant association between varying melasma severity with levels of thyroid stimulating hormone and free T4 (FT4), p = 0.375 and p = 0.208, respectively. The Janus II examination using polarized light modality has a weak positive correlation with the serum FT4 level (r = 0.3; p = 0.039). Weak correlation was also found between the two measurement strategies, Janus II and mMASI (r = 0.314; p = 0.03). BACKGROUND: Osteonecrosis most commonly affects the femoral head, especially in middle-aged adults. It can be caused by trauma, chronic inflammation, or infection. It leads to collapse of the entire femoral head and culminates with total hip replacement. CASE REPORT: A 29-year-old female with systemic lupus erythematosus (SLE) had a chief complaint of bilateral hip pain. She was diagnosed with early osteonecrosis of the femoral head (FICAT stage II) using magnetic resonance imaging and core decompression surgery was performed using three small diameter (4 mm) drillings and added biological treatment. She was evaluated with a visual analog scale (VAS), Harris hip score (HHS), and plain radiography in the pre-operative stage and post-operative follow-up. RESULTS: Functional outcome at 8-year follow-up showed improvement with significantly decreased VAS (pre-operative: 5, post-operative: 0), significant improvement of HHS from 52.725 points (poor) pre-operative to 92.025 points (excellent) post-operative, and subsided femoral head lesion. CONCLUSIONS: Surgical decompression and biological treatment result in decreased intraosseous pressure and enhanced osteogenesis. BACKGROUND: Melasma is a common hyperpigmentation disorder, which causes brownish discoloration of the face. Despite unclear mechanisms, thyroid hormones were thought to play a role in melasma. AIM: This study aims to determine and compare the clinical improvement of melasma in hyperthyroid patients receiving anti-thyroid drugs. METHODS: An interventional study with a quasi-experimental design (pre-post-intervention study) was conducted at the Internal Medicine Outpatient Clinic and Dermatology and Venereology Outpatient Clinic, Cipto Mangunkusumo Hospital, Jakarta, Indonesia, from July 2019 to March 2020. A comparative analysis was done to compare the modified melasma area and severity index (mMASI) in hyperthyroid patients before and after 12 weeks of anti-thyroid drugs. All subjects did not receive any additional topical therapies for their melasma. The clinical features were evaluated objectively at baseline and 12th-week visit, by mMASI score on different areas of the face (forehead, left-right malar, and chin) and Wood’s lamp examination. RESULTS: All areas showed a decline in mMASI score components (e.g., involvement areas and darkness degree) after 12 weeks of treatment. However, only the malar area showed a significant decline (p < 0.05). Wood’s lamp examination at baseline revealed dermal type melasma on 17 subjects, mixed type on six subjects, and epidermal type on one subject. All types remained unchanged after 12 weeks of treatment. CONCLUSIONS: Our study demonstrated that mMASI score in malar area improved significantly, this might be because malar area included this study were comprised of epidermal, dermal, and mixed type. On the other hand, based on Wood’s lamp examination, all types of melasma remained unchanged after 12 weeks of treatment. © 2021 Irma Bernadette S. BACKGROUND: Double-bundle (DB) anterior cruciate ligament reconstruction (ACLR) has been known to result in better functional outcomes, joint stability, and lower revision rates compared to single-bundle (SB) ACLR. However, given the increased invasiveness and damage to the surrounding tissue area, it is proposed that it may be associated with increased pain. AIM: This review aims to gather all studies and literature that reported pain as an outcome when comparing SB versus DB ACLR. METHODS: Literature searching was conducted across seven search engines for studies reporting pain as an outcome and comparing SB versus DB ACLR. RESULTS: Eight studies met the eligibility criteria and were included in the study. Overall, the studies show variable findings regarding pain in DB compared to SB ACLR, with the only statistically significant results from two studies indicating that DB ACLR is associated with more pain than SB ACLR. CONCLUSION: Based on the limited evidence available, no conclusions can be made regarding the pain experienced between people receiving either procedure. This constitutes a need for additional studies with increased follow-up time periods, larger sample size, and better study design. © 2021 Ludwig A. P. Pontoh, H. D. Diabetic Retinopathy (DR) can lead to vision loss if the patient does not get effective treatment based on the patient’s condition. Early detection is needed to know what an effective treatment for those patients is. For helping ophthalmologists, DR detection methods using computer-based were developed. Ophthalmologists can use the result of the method as a consideration in diagnosing the class of DR. One of the powerful methods is deep learning. The proposed method uses two deep learning architectures, namely Convolutional Neural Network (CNN) and Recurrent Neural Network (RNN), for DR detection. CNN is used to detect DR lesion features, and RNN is used for captioning based on those lesion features. We used three pre-trained CNN models, including AlexNet, VGGNet and GoogleNet, and used Long Short-Term Memory (LSTM) as RNN models. In the image preprocessing, we applied contrast enhancement using Contrast Limited Adaptive Histogram Equalization (CLAHE) and compared the results with those without CLAHE. We have done the training and testing process with a different proportion of data. The experimental results show that our proposed method can detect the lesion features and generate caption with the highest average accuracy of 96.12% for GoogleNet and LSTM with CLAHE and the proportion 70% training data 30% testing data. © 2021 the author(s). |

Diabetic Retinopathy (DR) is a long-term complication of Diabetes Mellitus (DM) that impairs vision. This stage occurs in visual impairment and blindness if treated late. DR identified through scanning fundus images. A technique on classifying DR in fundus images is the deep learning approach, one of the methods of implementing machine learning. In this study, the Convolutional Neural Networks (CNN) method applied with the ResNet-50 and DenseNet-121 architectures. The data adopted in this analysis was generated from DIARETDB1, an online database containing fundus images. Then, the pre-processing stage is carried out on the fundus image to improve model performance, such as selected the green channel from the images and inverted it, converted the images into grayscale images, and applied Contrast Limited Adaptive Histogram Equalization (CLAHE) for uniform contrast in the images. The outcome of this research indicates that the ResNet-50 model is better than DenseNet-121 in detecting DR. The most reliable results from the ResNet-50 model's case testing are accuracy, precision, and recall of 95%, 98%, and 96% respectively. © 2021, SCIK Publishing Corporation. All rights reserved. |

One of the most common neglected tropical skin diseases in Indonesia is scabies, commonly associated with overcrowding, poor hygiene and poor nutritional status. Boarding schools are at risk of harboring infected populations. As recurrence is common, non-medical personnel need to be involved in management of scabies. This study aimed to investigate how non-medical personnel, in this case, the boarding school teachers, could manage scabies in a boarding school in Indonesia. We trained the teachers using the education module for early scabies detection approach, namely DeSkab program, to be able to identify three major signs of scabies in the students. Impact of this approach was evaluated in 2017 and 2018 at a religious boarding school in Bogor, Indonesia involving 127 and 202 students, consecutively. An education module was given to 16 boarding school teachers expecting them to be able to examine students prior to school admission and before and after mid-semester break. Teachers’ knowledge increased significantly after the training (p <0.001). This knowledge improvement is an important basis for teachers as non-medical personnel to implement independent scabies monitoring process during the educational year to find students with suspected scabies. Those suspected scabies students were directly referred to nearby public health clinic and treated with 5% permethrin and re-treated one week later. Prevalence of scabies decreased from 76% in 2017 to 49% in 2018 in 69 students who can be followed-up in two consecutive years (p-value = 0.014). Knowing the species of nontuberculous mycobacteria (NTM) causing disease in a patient population can guide empiric treatment. In this study we aimed to determine the species of NTM causing disease in a selected patient population in Indonesia in order to guide empiric treatment. We retrospectively reviewed the records of all patients treated at 5 hospitals in Jakarta, Indonesia, diagnosed with NTM infection during January 2018-December 2019. The inclusion criterion for study subjects was having a positive test for NTM and the exclusion criterion was having a positive culture for Mycobacterium tuberculosis complex. Study subjects were confirmed to have NTM by a MPT64 rapid test. The species of NTM were identified by DNA sequencing of Mycobacterium 16S rRNA. A total of 15 patients were determined to have NTM at the study institutions during the study period, 53% male. The average age of study subjects was 45 (range: 4-83) years. The NTM positive specimens were obtained from the sputum (n = 5), skin and soft tissue biopsies (n = 5), and lymphoid tissue biopsies (n = 5). Of the 15 study subjects, 8 (53%) had M. abscessus, 5 (33%) had M. intracellulare, 1 (7%) had M. avium and 1 (7%) had M. cookii. Of the 5 NTM isolates from the sputum, 3 isolates were M. abscessus, 1 isolate was M. intracellulare and 1 isolate was M. Cookii. Of the 5 NTM isolates from skin and soft biopsies, 3 isolates were M. abscessus and the other 2 isolates were M. intracellulare. Of the 5 NTM isolates from lymphoid tissue biopsies, 2 isolates were M. abscessus, 2 isolates were M. intracellulare and 1 isolate was M. Avium. In our study, the most common NTIM isolate was M. abscessus. Empiric treatment of NTM in the study population should include coverage of M. abscessus. Further studies with larger numbers of subjects from multiple institutions are needed to determine if these findings hold true for other parts of Indonesia. © 2021, SEAMEO TROPMED Network. Background: Invasive breast carcinoma of no special type (IBC-NST) is the most common type of breast cancer and mainly causes regional lymph-node metastasis (LNM). We investigated the potential for AKT2 expression as a predictive biomarker for LNM in IBC-NST. Methods: Forty-eight paraffin blocks containing IBC-NST primary tumors were divided into two groups based on presence or absence of LNM. Age, tumor grade, tumor size, lymphovascular invasion (LVI), and AKT expression were assessed. AKT2 expression was assessed based on immunohistochemical staining, while other data were collected from archives. Results: Multiple logistic regression results showed that AKT2 expression and LVI were significantly associated with LNM (odds ratio [OR], 5.32; 95% confidence interval [CI], 1.42 to 19.93 and OR, 4.46; 95% CI, 1.17 to 16.97, respectively). AKT2 expression was able to discriminate against LNM (area under the receiver operating characteristic, 0.799 ± 0.063; 95% CI, 0.676 to 0.921) at an H-score cutoff of 104.62 (83.3% sensitivity, 62.5% specificity). Conclusions: AKT2 expression has potential as a predictor of LNM in IBC-NST. The H-score cutoff for AKT2 expression can be used as a classification guide in future studies. © 2021 The Korean Society of Pathologists/The Korean Society for Cytopathology. |

Ovarian yolk sac tumor in pregnancy is a very rare case (<5%). The management could be very challenging since studies regarding the disease are very limited. This case report is written in order to report a rare case of yolk sac tumor in pregnancy and its management. A 29-year-old woman with a 16 weeks gestational age (WGA) in her first pregnancy presented in the emergency room with severe lower abdominal pain. Next, she underwent exploratory laparotomy, and a biopsy was performed, which indicated an ovarian yolk sac tumor. The patient was then given neoadjuvant chemotherapy with carboplatin and paclitaxel. The pregnancy resulted in an intrauterine growth restriction (IUGR) baby, delivered on 33 WGA. The baby was delivered through C-section and the mother continued to undergo optimally debulked laparotomy, total hysterectomy, bilateral salphingo-oophorectomy, omentectomy, and rectosigmoid tumor resection. In dealing with a rare case with limited resources, tailor-made management is required. The most ideal treatment may not be performed, but the clinician should be more adaptive for the patient to have a better outcome. © 2021. |
