A thyroid storm (or thyroid crisis) is an emergency in endocrinology. It is a form of complication of hyperthyroidism that can be life-threatening. Inadequate control of hyperthyroidism in pregnancy could develop into thyroid storm, especially in the peripartum period. We present a woman came in the second stage of labour, with thyroid storm, superimposed pre-eclampsia, acute lung oedema and impending respiratory failure. Treatment for thyroid storm, pre-eclampsia protocol and corticosteroid was delivered. The baby was born uneventfully, while the mother was discharged after 5 days of hospitalisation. Delivery is an important precipitant in the development of thyroid storm in uncontrolled hyperthyroidism in pregnancy. Although very rare, it can cause severe consequences. Diagnosis and treatment guidelines for thyroid storm were available and should be done aggressively and immediately. Uncontrolled hyperthyroidism should be prevented by adequate control in thyroid hormone levels, especially before the peripartum period. © |

Indonesia is facing increasing stroke prevalence in the past 5 years. Ischemic stroke imposes economic and productivity burden if it is not addressed properly. Endovascular treatment studies are conducted in developed countries where facilities and cost do not count in therapy consideration if it is indicated. Developing countries like Indonesia should work hard to provide the best hyperacute stroke care with protocol deviation and limitation. This is the first review on endovascular treatment practice in a top single-center hospital in Indonesia. Further improvement is needed to catch up with state-of-the-art hyperacute ischemic stroke treatment. © 2021 |

Nitric Oxide (NO) is an important mediator in the inflammatory and immune systems. The eNOS gene is one of the three isoforms of Nitric Oxide Synthase (NOS), which is responsible for synthesizing NO. Periodontitis is an inflammatory disease in periodontal tissue with genetic involvement. Polymorphism in eNOS gene changes the functional aspect of this gene and is associated with several inflammatory diseases including periodontitis. Aim: To detect Endothelial Nitric Oxide Synthase intron 4 gene polymorphism in Indonesian population with periodontitis. Analysis of the Endothelial Nitric Oxide Synthase (eNOS) intron 4 gene polymorphism was observed by carrying out PCR method followed by electrophoresis for the analysis, without the usage of restriction enzyme. The chi-square test and odds ratio were performed for statistical analysis. In this study, there were 34 samples with AA genotype, 3 samples with AB genotype, and 13 samples with BB genotype in periodontitis group. Whereas in the control group, there were 41 samples with AA genotype and 9 samples with BB genotype. AB genotype was absent in the control group. In periodontitis group, there were 71 A alleles and 29 B alleles, and in the control group, 82 A alleles and 18 B alleles were found. Polymorphic genotypes and alleles were found higher in periodontitis sample (32% and 29%) than healthy controls (18%). The polymorphism of eNOS intron 4 was found in periodontitis patients. There is no significant distribution difference was found between the periodontitis patients and the control group. Vitamin D receptor (VDR) is included in the type of protein that serves as the biological function regulator of vitamin D. Tooth formation, especially in enamel and dentin calcification, as well as maintaining the balance of phosphate and calcium ions which is an important factor in protecting teeth requires support from vitamin D. The VDR gene will regulate the activity of VDR proteins. Caries is a multifactorial disease in which genetic factors can affect the host susceptibility to caries. Polymorphism in the VDR gene is suspected to affect the host susceptibility to caries through changes in calcium metabolism. This study aims to discover the VDR gene polymorphism and its association with caries patients in Indonesia. 100 DNA samples from 100 blood samples, including 50 dental caries patients and 50 healthy controls, were analyzed using PCR-RFLP technique. PCR products were digested with the TaqI restrictive enzyme, then assessed with statistical analysis using Fisher's exact test and Continuity correction test. In the caries group, there were no samples with CC genotype, 4 samples with CT genotype, and 46 samples with TT genotype. There were also 4 C alleles and 96 T alleles. Polymorphic genotypes and alleles were found higher in the caries group (100% and 96%) than healthy controls (88% and 84%). These results conclude that the polymorphism of VDR TaqI (rs731236) gene was found in patients with dental caries. The distribution of genotypes and allele distributions of VDR TaqI (rs731236) gene between caries and healthy controls significantly differs noticeable (p <0.05). © Published under licence by IOP Publishing Ltd. |

Objectives: To investigate the safety and pharmacokinetic profiles of long-acting injectable pre-exposure prophylaxis (LAI PrEP), notably cabotegravir (CAB-LA) and rilpivirine (RPV-LA), for the prevention of human immunodeficiency virus-1 (HIV-1) infection. Methods: Eligible randomized trials of LAI PrEP in HIV-uninfected and/or healthy patients were included and assessed with the Revised Cochrane risk-of-bias tool for randomized trials. Where feasible, a meta-analysis was performed for safety outcomes by using a random-effects model with risk ratios and their 95% confidence intervals as the common effect measure. The protocol was registered with PROSPERO CRD42020154772. Results: Eight studies cumulating a total of 666 participants were included in this systematic review, including five (362 intervention-arm volunteers) and four trials (194 intervention-arm volunteers) that investigated CAB-LA and RPV-LA, respectively. We found that both CAB-LA and RPV-LA were generally well-tolerated as their safety profiles were similar to placebo in terms of any adverse event (AE), serious AE, and AE-related withdrawals. Furthermore, pharmacokinetic analyses revealed favorable prospects in viral inhibitory activity of CAB-LA and RPV-LA. Intramuscular (IM) injection of CAB-LA 600 mg Q8W was superior to CAB-LA 800 mg Q12W in male participants, while the same was true for RPV-LA 1200 mg IM Q8W over other dosing regimens. Although these results are promising, further research is required to confirm the findings on RPV-LA as current evidence is limited. Conclusion: CAB-LA and RPV-LA have promising safety and pharmacokinetic profiles. Investigating causation is a primary goal in forensic/legal medicine, aiming to establish the connection between an unlawful/negligent act and an adverse outcome. In malpractice litigation involving a healthcare-associated infection due to a failure of infection prevention and control prac-tices, the medicolegal causal analysis needs to quantify the individual causal probabilities to meet the evidentiary requirements of the court. In this paper, we present the investigation of the most probable cause of bacterial endocarditis in a patient who underwent an invasive procedure at a dental/oral surgical practice where an outbreak of bacterial endocarditis had already been identified by the state Department of Health. We assessed the probability that the patient’s endocarditis was part of the outbreak versus that it was an unrelated sporadic infection using the INFERENCE (Integration of Forensic Epidemiology and the Rigorous Evaluation of Causation Elements) approach to medicolegal causation analysis. This paper describes the step-by-step application of the INFERENCE approach to demonstrate its utility in quantifying the probability of causation. The use of INFERENCE provides the court with an evidence-based, transparent, and reliable guide to determine liability, causation, and damages. © 2021 by the authors. BACKGROUND: This study aimed to find out the incidence and characteristics of musculoskeletal injuries among Indonesian volleyball athletes during a national volleyball training and championship. METHODS: A prospective cohort study involving 34 Indonesian volleyball national team athletes (18 male and 16 female) during one volleyball national training and championship in July-September 2019 was conducted. All athletes were observed for 11 weeks and the incidence and the characteristics (duration, pain level, severity in loss time, nature, recurrence, anatomical location and types) of the injuries were documented. Some etiological factors of the injuries such as the court zone (back, front/net, and un-specified) time of injury (practice, competition or both), injury mechanism (ball contact, player contact, surface contact, non-contact or overuse), biomechanics (bending, compression, repetitive or traction) and movements (blocking, digging, passing, serving, spiking, or not specific) were also recorded. Incidence of injuries was presented in 1000 player hours. RESULTS: The incidence of injuries was 20.4/1000 player hours. Only one out of the 34 athletes did not have injury. Injuries occurred most frequently in the knee (36%), followed by the ankle (18%) and waist (16%). The most common type of injuries was sprain (21%) and tendinitis (21%) followed by low back pain (13%). The overuse and recurrence type of injuries were much more dominant to the acute one. CONCLUSIONS: Musculoskeletal injuries were common among Indonesian volleyball athletes. This adds information to the limited available evidence concerning the incidence, prevalence and etiology of volleyball-specific injuries which were needed to prevent its occurrence. © 2021 Edizioni Minerva Medica. The use of pacemakers is necessary for patients with symptomatic bradycardia. Pacemaker implantation also acts as a life-saving procedure. However, there are several reports that patients with a pacemaker (the most widely used pacemaker is on the right ventricle, known as single chamber pacemaker) had disturbances in left ventricle contraction lead to left ventricular systolic dysfunction. Global Longitudinal Strain (GLS) Echocardiography can confirm these left ventricular disturbances. Echocardiography examination is best carried out before and after single-chamber PPM implantation. This study compares PPM placement in apical Right Ventricle (RV) and Right Ventricular Outflow Tract (RVOT) and compares paced QRS duration 150 ms and > 150 ms. Pacing burden >40% causes subclinical left ventricular systolic dysfunction after a month of PPM implantation with decreased GLS in apical RV pacing.This study gives additional information that PPM placement is preferably in RVOT with some specific settings. However, there are many ways to reduce the effects of impaired left ventricular function due to lead installation on the PPM single-chamber device. By using the PPM device which has a dual chamber, the leads will be placed in RVOT and also in the right atrium with atrial-ventricle synchronization. The aim is to avoid impaired left ventricular pump function as supported by a systematic review which states the superiority of dual-chamber PPM over single-chamber PPM in reducing atrial fibrillation and pacemaker syndrome. In the importance of quality of life related to heart disease, the Indonesian version of the MacNew questionnaire can be used. This questionnaire seeks the quality of life of patients with coronary heart disease after revascularization surgery and has undergone rehabilitation. Another important thing is the suitability of the adaptation to the original English version. |

BACKGROUND: Folic acid (FA) and vitamin B12 treatment have been routinely prescribed to lower serum homocysteine levels and to reduce inflammation. However, no study has been conducted to determine serum folic acid (SFA) and vitamin B12 (B12) levels in patients who have twice-weekly hemodialysis. The aim of our study was to assess serum folate and B12 levels in chronic hemodialysis patients and their relationship with hsCRP and homocysteine levels. METHODS: Our study was a cross-sectional study involcing patients who had twice-weekly hemodialysis in Dr Cipto Mangunkusumo National Hospital Jakarta, Indonesia. Predialysis blood samples were taken to measure SFA, B12, homocysteine and hsCRP levels. Patients with medical conditions affecting the assays were excluded. Spearman correlation was used to compare variables. RESULTS: Eighty subjects enrolled in this study. Among those of non-given folic acid and vitamin B-12 supplementation, only 3.85% of subjects had low folic acid levels, and none had low vitamin B12 levels. A moderate negative correlation between serum folic acid and homocysteine level (p≤0.001; r=-0.42) and a weak correlation between serum vitamin B12 and homocysteine level (p=0.009; r=-0.29) was found. Among the high-risk cardiovascular group (CRP>3, n=49), there is a moderate negative correlation between serum folic acid and homocysteine level (p≤0.001; r=-0.561) and a weak negative correlation between vitamin B12 and homocysteine level (p=0.018; r=-0.338). CONCLUSION: There is a significant negative correlation between serum vitamin B12 and folic acid with homocysteine levels, especially in high-risk cardiovascular group. |

BACKGROUND: Cardiac iron toxicity is a major cause of mortality in transfusion-dependent beta-thalassemia major patients. The main modality for detecting cardiac iron toxicity is MRI T2* with limited availability. This study aims to obtain iron toxicity profiles in transfusion-dependent beta-thalassemia patients; to see a correlation between iron toxicity and cardiac function. METHODS: We conducted a cross-sectional study at the Adult Thalassemia Polyclinic of Cipto Mangunkusumo Hospital, Indonesia from December 2017 to March 2018. We performed the statistical analysis using Pearson/Spearman Test comparing MRI T2* values with ejection fraction and E/A ratio. RESULTS: The median of 4-months mean of ferritin levels was 5130 ng/mL. The mean for cardiac T2* was 24.96 ms. Severe cardiac hemosiderosis (mean cardiac MRI T2* < 10 ms) occurred in 11.3% of the subjects. There was weak correlation between serum ferritin with cardiac iron toxicity (r=-0.272, p=0.032) and ejection fraction (r=-0.281, p=0.013). But, there was no correlation between cardiac iron toxicity with ejection fraction and E/A ratio. CONCLUSION: Serum ferritin correlated weakly with cardiac iron toxicity and cardiac systolic function. Meanwhile, there was no correlation between cardiac iron toxicity with cardiac systolic and diastolic function. |
