Objectives: A chronic complication of type-2 diabetes mellitus (DMT-2) is Diabetic Foot Ulcer (DFU). The main treatment used in DFU is wound cleansing, followed by dressing the wound as a local control to increase tissue granulation and epithelialization. This study aims to compare the efficacy of the combination of Hyaluronic Acid with Platelet Rich Fibrin (HAPRF) and Platelet Rich Fibrin alone in DFU. Methods: We conducted a randomized controlled trial from July 2019 to April 2020. The study was approved by the Ethics Committee of the Faculty of Medicine of Universitas Indonesia ID 0855 / UN2.F1 / ETIK / 2018. Informed consent was obtained from the patients. This was a randomized clinical study to compare the efficacy of HAPRF and PRF in DFU one week post debridement. Twenty DFU samples were collected divided into 2 treatment groups: topical treatment using HAPRF compare with PRF alone. Assessment for wounds improvement was recorded using a digital camera 48 mega pixel with an accuracy of 0.1% on day-0, 3, 7, and 14. The results of the wound photographs were processed using ImageJ software. The granulation area (GA) and wound area (WA) were evaluated by IBM SPSS software v.20. The general data description was presented in median (range) value and parameter’s differences were conducted using Mann–Whitney test Results: The two treatment groups showed insignificant difference in characteristics between both group before intervention. The mean granulation width after two weeks of use HAPRF was 97.4% and PRF was 81.9%. Statistical analysis using Mann Whitney test showed granulation area of HAPRF group was significantly different compared with PRF group on day-3(p=0.047), day-7 ( p = 0.004) and day-14 ( p < 0.001). At the end of the wound healing process, the HAPRF group was significantly different compared with PRF group on Δ day 0−3 ( p=0.048), Δ day 0−7 ( p = 0.039), and Δ day 0−14 ( p = 0.023). Conclusions: HAPRF improves wound healing rate through increasing granulation tissue and epithelialization compared with PRF only in diabetic foot ulcer after 2 weeks post debridement compared to PRF. © 2021, Jordan Journal of Biological Sciences. Objective: Post-stroke urinary incontinence (UI) is one of the sequelae of stroke. This situation affects all aspects of the patient's life - physically, psychologically, socially, and spiritually. This study aimed to investigate the experience of patients' success in facing a post-stroke UI. Methods: A qualitative study using the Rapid Assessment Procedure (RAP) approach was used in this study. Informants were selected using purposive sampling. In-depth interviews with as many as 8 patients who had recovered from post-stroke UI and living in the greater area of Southeast Sulawesi (Indonesia) were conducted. In-depth interviews were also conducted with 8 caregivers and 2 nurses. Data were analyzed using a thematic analysis approach and interpretation of data was based on Humanbecoming theory and Self-care deficit theory of nursing. Results: Five successful things the patients experienced during post-stroke UI were identified. The five successes were as follows: they provided information to get to know and understand post-stroke UI, followed the procedures to overcome post-stroke UI, conducted self-control exercises and stayed motivated, performed daily activities independently according to ability, and made use of family support and peers' attention. Conclusions: These findings indicated that persistence, belief, independence, and social support (family and peer) made patients to successfully face their post-stroke UI and improved their quality of life. These findings also became the basis for developing a post-stroke UI management model based on Humanbecoming theory and Self-care deficit theory of nursing. © 2021 Heltty Heltty et al., published by Sciendo. |

In this era of COVID-19, suspected breast cancer patients experience delay in diagnosis due to the fear of contracting the virus and reduction of non-COVID-19 health services. Furthermore, it may lead to potential increase in the incidence of advanced cancers in the future. Ultrasound-guided (US-guided) percutaneous core needle biopsy (CNB) is a great option for the diagnosis of cancer but it is poorly utilized. This study aimed to prove that the US-guided CNB is accurate when performed in a local setting and a potential solution for diagnosing breast cancer patients in this pandemic. In addition, it was a single health center cross-sectional study, and the participants were all breast cancer patients that had US-guided CNB from 2013-2019. The pathology results from US-guided CNB were compared to specimens from post-CNB surgeries. The data were collected from medical records and the immunohistochemistry (IHC) examinations were carried out for malignancy. There were 163 patients who were included in this study, 86 had malignancies and 77 had benign tumor reported in their CNB results. The US-guided CNB had 100% sensitivity and specificity compared to surgery. With its lower cost, time usage, and patient exposure to the hospital environment, US-guided CNB should replace open surgery biopsy for diagnosing suspicious breast cancers during the pandemic in Indonesia. Objective: To determine whether thrombocytosis is a prognostic factor for epithelial ovarian cancer and its relationship with 3-year overall survival in epithelial ovarian cancer patients. Methods: This study is a retrospective cohort study using medical record of patients with epithelial ovarian cancer registered in cancer registry of Oncology Division in Obstetrics and Gynecology Department, Dr. Cipto Mangunkusumo National General Hospital from January 2014 - July 2016. Data were collected when subjects were first until diseases outcomes identified in 3 years. Result: Out of 220 subjects, 132 (60%) were patients with advanced stage epithelial ovarian cancer (stage II/III/IV). 94 (42.7%) subjects had thrombocytosis. Patients with advanced stage of disease had higher risk of having thrombocytosis than the ones with earlier stage (p=0.005; OR=2.329). Correlation between thrombocytosis and 3-year overall survival was known to be insignificant (p=0.555). There was shorter mean time survival between patients with thrombocytosis and the ones without but the there was no significant difference in hazard ratio between the two groups (p = 0.399). Conclusions: Thrombocytosis is not a prognostic factor in patients with epithelial ovarian cancer. Objective: To determine the quality of antenatal care received by pregnant women, and perceptions of pregnant women and antenatal care providers regarding antenatal care. Methods: A quantitative and qualitative study of cases of referred pregnant women in the Emergency Room (ER) of Dr. Cipto Mangunkusumo Hospital (RSCM) from 2017-2018 was conducted. Quantitative data was obtained by assessing the overall completeness of filling in the Maternal Child Health (MCH) book used to report the antenatal care received by the subjects. The completeness of antenatal care in the health facility was obtained using a checklist. Qualitative data was obtained by interviewing pregnant women and antenatal care providers in health facilities. Result: There were 1.442 cases of referred pregnant women in RSCM's ER for the year 2017-2018, of whom 820 possessed and could show their MCH Book. Pregnancy in adolescence below 20 years (9.1%) and pregnancy after age 35 (19.5%) were reported. Most pregnant women were well educated (74.3%), referred from hospital or clinic (57.6%), received antenatal care in a public health center (38.7%), were in labor (32%), and were in the third trimester of pregnancy (92%). All subjects presented an incomplete MCH book. Almost half had inadequate antenatal frequency (46%). The completeness of antenatal care components in health care was 90-100%. Qualitatively, the perceptions of mothers and antenatal care providers regarding quality of antenatal care, was inadequate. Conclusions: The quality of antenatal care in the case of referred pregnant women at the RSCM's ER based on MCH Book records did not meet the required standard. © Creative Commons Atribuição-Não Comercial 4.0 Internacional |

Objective: To determine the evidence about timing and role of laparoscopy in ascites work-up. Methods: : Case report Case: A-26-year-old P2 woman went to our hospital with a history of vaginal delivery 22 days before admission. She also presented with massive ascites. No obstetric complication was found. Transvaginal ultrasound revealed normal postpartum uterus and ovaries, surrounded with ascites. Both the liver and kidneys were found normal on ultrasound. Abdominal CT scan with contrast showed massive ascites with thickened omentum. Ascites ADA (adenosine deaminase) was 36 IU/L. Diagnostic laparoscopy was performed, we found massive yellowish ascites, miliary whitish lesions and hyperaemic tubal enlargement with thickening of the peritoneum. Biopsy was taken, pathology examination showed the appearance of chronic salpingitis and granulomatous peritonitis, in accordance with tuberculosis peritonitis. Conclusions: Laparoscopy should be the method of choice in diagnosing ascites with unknown etiology after inconclusive results of laboratory and radiological examination. Better visualization, typical appearance in peritoneal TB, chance to perform direct biopsy with lower risk of morbidity are the hallmarks of laparoscopy to be the method of choice to rule out the etiology of ascites. © 2021 Indonesian Journal of Obstetrics and Gynecology. The use of pacemakers is necessary for patients with symptomatic bradycardia. Pacemaker implantation also acts as a life-saving procedure. However, there are several reports that patients with a pacemaker (the most widely used pacemaker is on the right ventricle, known as single chamber pacemaker) had disturbances in left ventricle contraction lead to left ventricular systolic dysfunction. Global Longitudinal Strain (GLS) Echocardiography can confirm these left ventricular disturbances. Echocardiography examination is best carried out before and after single-chamber PPM implantation. This study compares PPM placement in apical Right Ventricle (RV) and Right Ventricular Outflow Tract (RVOT) and compares paced QRS duration 150 ms and > 150 ms. Pacing burden >40% causes subclinical left ventricular systolic dysfunction after a month of PPM implantation with decreased GLS in apical RV pacing.This study gives additional information that PPM placement is preferably in RVOT with some specific settings. However, there are many ways to reduce the effects of impaired left ventricular function due to lead installation on the PPM single-chamber device. By using the PPM device which has a dual chamber, the leads will be placed in RVOT and also in the right atrium with atrial-ventricle synchronization. The aim is to avoid impaired left ventricular pump function as supported by a systematic review which states the superiority of dual-chamber PPM over single-chamber PPM in reducing atrial fibrillation and pacemaker syndrome. In the importance of quality of life related to heart disease, the Indonesian version of the MacNew questionnaire can be used. This questionnaire seeks the quality of life of patients with coronary heart disease after revascularization surgery and has undergone rehabilitation. Another important thing is the suitability of the adaptation to the original English version. |

BACKGROUND: Folic acid (FA) and vitamin B12 treatment have been routinely prescribed to lower serum homocysteine levels and to reduce inflammation. However, no study has been conducted to determine serum folic acid (SFA) and vitamin B12 (B12) levels in patients who have twice-weekly hemodialysis. The aim of our study was to assess serum folate and B12 levels in chronic hemodialysis patients and their relationship with hsCRP and homocysteine levels. METHODS: Our study was a cross-sectional study involcing patients who had twice-weekly hemodialysis in Dr Cipto Mangunkusumo National Hospital Jakarta, Indonesia. Predialysis blood samples were taken to measure SFA, B12, homocysteine and hsCRP levels. Patients with medical conditions affecting the assays were excluded. Spearman correlation was used to compare variables. RESULTS: Eighty subjects enrolled in this study. Among those of non-given folic acid and vitamin B-12 supplementation, only 3.85% of subjects had low folic acid levels, and none had low vitamin B12 levels. A moderate negative correlation between serum folic acid and homocysteine level (p≤0.001; r=-0.42) and a weak correlation between serum vitamin B12 and homocysteine level (p=0.009; r=-0.29) was found. Among the high-risk cardiovascular group (CRP>3, n=49), there is a moderate negative correlation between serum folic acid and homocysteine level (p≤0.001; r=-0.561) and a weak negative correlation between vitamin B12 and homocysteine level (p=0.018; r=-0.338). CONCLUSION: There is a significant negative correlation between serum vitamin B12 and folic acid with homocysteine levels, especially in high-risk cardiovascular group. |

BACKGROUND: Cardiac iron toxicity is a major cause of mortality in transfusion-dependent beta-thalassemia major patients. The main modality for detecting cardiac iron toxicity is MRI T2* with limited availability. This study aims to obtain iron toxicity profiles in transfusion-dependent beta-thalassemia patients; to see a correlation between iron toxicity and cardiac function. METHODS: We conducted a cross-sectional study at the Adult Thalassemia Polyclinic of Cipto Mangunkusumo Hospital, Indonesia from December 2017 to March 2018. We performed the statistical analysis using Pearson/Spearman Test comparing MRI T2* values with ejection fraction and E/A ratio. RESULTS: The median of 4-months mean of ferritin levels was 5130 ng/mL. The mean for cardiac T2* was 24.96 ms. Severe cardiac hemosiderosis (mean cardiac MRI T2* < 10 ms) occurred in 11.3% of the subjects. There was weak correlation between serum ferritin with cardiac iron toxicity (r=-0.272, p=0.032) and ejection fraction (r=-0.281, p=0.013). But, there was no correlation between cardiac iron toxicity with ejection fraction and E/A ratio. CONCLUSION: Serum ferritin correlated weakly with cardiac iron toxicity and cardiac systolic function. Meanwhile, there was no correlation between cardiac iron toxicity with cardiac systolic and diastolic function. |
