Postmenopausal hypoestrogenism is associated with skin aging, for which phytoestrogen derived from the seeds of Trigonella foenum graecum (fenugreek) is expected to be an alternative solution to reduce wrinkles and increase the thickness of postmenopausal women’s skin. This study was a randomized, double-blind, controlled 12-weeks trial conducted at a general hospital in Jakarta, Indonesia, from January to November 2019. Subjects were 50 postmenopausal women divided into two groups: the intervention group was given 5% fenugreek cream and base cream was given to the placebo group. The results of the independent t-test showed that both groups were able to achieve statistically significant improvement in wrinkle scores on the forehead, crow’s feet, and nasolabial folds which was assessed by photography scoring based on Bazin’s skin aging atlas for Asian skin volume 2 but there was no significant disparity between both groups at all time points. Dermal thickness was assessed by high-resolution ultrasound GE LOGIQ E9 (18 MHz), which showed similar results for both groups with significant improvements in the 8th week compared to baseline and a significant decrease by the 12th week. No significant differences were observed in the skin wrinkle score and skin thickness after the application of fenugreek cream compared to placebo.We suspect that a concentration of 5% was not adequate for the expected antiskin aging effects. Further studies are necessary to determine a more appropriate fenugreek concentration to permit clinical use as an antiskin aging therapy in postmenopausal women. © 2021 Shannaz Nadia Yusharyahya et al. A previous study showed that alpha-mangostin (AM) showed benefit when given to sorafenib (SOR)-surviving cells. However, the mechanism was not fully understood. The present study aimed to understand the effect of AM on SOR-surviving cells and its agent concerning drug transporters. SOR-surviving cells were treated with SOR 10 μM. Surviving cells were divided into four groups of treatment, namely, vehicle only dimethyl sulfoxide (DMSO), SOR 10 μM, AM 20 μM, or combination of SOR 10 μM-AM 20 μM. As controls, HepG2 naïve cells were treated with DMSO only or AM 20 μM. Cell viability was counted using trypan blue exclusion assay. Simultaneously, the mRNA expressions of P-glycoprotein (P-gp), ABCG2, MRP2, MRP3, OCT1, and OATP1B3 drug transporters were examined with quantitative reverse transcriptase-polymerase chain reaction. Decreased mRNA expression of P-gp was found in SOR-surviving cells treated with SOR. In contrast, AM alone or SOR's combination caused a significant increase in both efflux and influx transporters, no difference in fold increase of all transporters evaluated in AM versus SOR-AM combinations. Breast cancer is one of the most diagnosed cancers in women; the number of cases continues to rise. The high prevalence and increased incidence need more attention in developing effective therapy. Current passive therapy has several drawbacks that have not yet been resolved. Thus, an alternative and preventive therapy for cancer is needed by utilizing vaccines. Immunoinformatics approach is one of the promising methods predicting epitopes in vaccine research. This approach could accelerate the initial study process of vaccine development and reduce research costs. Epitope conservancy and vaccine coverage are important parameters in vaccine research due to addressing the variability and diversity of cancer genomics. This study will be carried out on the multiepitope characterization of potential T cells against the protein mechanism in breast cancer. Proteins used in this study are Mucin-4, Phosphatase And Tensin Homolog, and Receptor tyrosine-protein kinase erbB-2. CTL epitopes, antigenicity, immunogenicity, allergenicity, and toxicity were predicted for the peptide vaccine. Immunoinformatics analysis generates a multiepitope sequence consisting of seven epitopes: DPVALVAPF, SVAYRLGTL, SQINTLNTL, RFRELVSEF, VTSANIQEF, RPRFRELVS, and MYFEFPQPL by AAY linker. The docking and molecular dynamics analyses were conducted to confirm the interactions between the multiepitope vaccine molecule and TLR-4-MD. Tyrosine kinase inhibitors (TKIs; e.g., erlotinib, gefitinib, and afatinib) are the first-line therapy for non-small-cell lung cancer (NSCLC) patients with epidermal growth factor receptor (EGFR) (+) common mutation. The study’s objective was to analyze the cost-effectiveness of erlotinib, gefitinib, and afatinib in NSCLC patients. The subjects of the study were NSCLC patients with EGFR (+) mutation receiving either erlotinib, gefitinib, or afatinib from January 2017 to December 2019. The exclusion criteria were patients receiving the respective therapy for less than 2 months and patients unable to complete the treatment until after December 2019. The parameter of treatment effectiveness was progression-free survival (PFS), which was measured as the time from initiation of the therapy until disease progression occurred or a patient became deceased. Direct medical costs, from the hospital perspective, were calculated during the treatment. A nonparametric Kruskal-Wallis test was conducted to compare the median PFS and direct medical cost between the three treatment groups. The median PFS of patients receiving erlotinib, gefitinib, and afatinib was 8 months, 12 months, and 5 months, respectively. There were significant differences in the monthly direct medical costs between the study groups: erlotinib (IDR 13,545,116), gefitinib (IDR 14,727,887), and afatinib (IDR 12,146,834). The cost-effectiveness ratio of the study groups was as follows: erlotinib IDR 1,693,139.50/months; gefitinib IDR 1,227,323.92/months; and afatinib IDR 2,429,366.80/months. Background: The World Health Organization reported Indonesia as one of the countries with the most prevalent cases of diphtheria worldwide. The microbiological aspects of diphtheria-inducing bacteria are of great significance in tracing disease transmission and case management. However, clinical aspects are critical for updating clinical features and case management in the field, which may sometimes differ from theoretical foundations. Objectives: This study aimed to identify the microbiological and clinical aspects, including molecular typing and case fatality rates, in diphtheria-confirmed cases from the capital city of Indonesia, Jakarta, and surrounding areas in 2017. Methods: The microbiological aspect of 40 diphtheria-confirmed cases were obtained by re-identify diphtheria-inducing bacteria isolated from the samples, while the clinical aspects of the cases were obtained from the medical records and epidemiological data. The chi-square test was used to examine the correlation between fatal cases and myocarditis and diphtheria antitoxin administra-tion delay. In this study, P ≤ 0.05 was set as the significance level. Results: All 40 diphtheria confirmed cases were induced by toxigenic Corynebacterium diphtheriae with two biotypes, namely inter-medius (60.0%) and mitis (40.0%). There are six sequence types of bacteria with two main sequence types, ie, ST534 (46.4%) and ST377 (35.7%). The proportions of cases that had a fever and sore throat were 72.5% and 77.5%, respectively; however, the prevalence rates of the cases with pseudomembrane and bull neck were 100% and 47%, respectively. Most cases were administered a combination of penicillin or erythromycin with other antibiotics (40%), and 22.5% of the cases only received penicillin. Myocarditis was noticed in three fatal cases, and their relationship was statistically significant (P = 0.000). All five fatal cases (12.5% of cases) received diphtheria antitoxin (DAT) lately or had not received it yet. Conclusions: Toxigenic C. diphtheriae with two biotypes (namely mitis and intermedius) and two main sequence types (ie, ST534 and ST377) was the causative agent of diphtheria-confirmed cases from Jakarta and surrounding areas in 2017. Objective: Alzheimer's disease (AD) is a progressive neurodegenerative disorder characterized by memory deficits and other cognitive disorders, which can be accompanied by personality changes. Long-term use of medications available to treat AD today have a variety of side-effects. Acupuncture, as a nonpharmacologic therapeutic modality providing stimulation at acupuncture points, using filiform needles, has been widely tested and used to manage of AD and can be a therapeutic option, considering its effectiveness and lack of side-effects. Methods: This literature review examines the role of acupuncture in AD treatment. Results: Acupuncture can ameliorate AD symptoms through decreasing amyloid-β protein, reducing neuroinflammation, enhancing the antioxidant system, improving neurogenesis, enhancing prosurvival protein, reducing proapoptotic protein, and regulating brain energy metabolism. Conclusions: According to various research findings, acupuncture may be a therapeutic choice for addressing AD that avoids the long-term side-effects caused by medical therapy. © 2021, Mary Ann Liebert, Inc., publishers. |

Background: Third-molar extraction is a common oral surgical procedure that can cause complications. Although rare, nerve injuries that result in permanent neuropathy can occur and include a variety of complaints such as sensory disorders, taste-sensation disorders, speech articulation disorders, etc. Acupuncture is one of several nonpharmacologic therapies that has played a role in managing neuropathic lesions and has been proven to produce good results. Case: A 44-year-old woman with postodontectomy neuropathy developed paresthesia, dysarthria, xerostomia, dysgeusia, pain in the gums and lower right jaw, a chewing disorder, and cephalgia. Her numerical rating scale (NRS) results were: lower right gum pain, 3/10; numbness of the tongue, 4/10; and headache, 1/10. A physical examination revealed dysarthria, a decreased sense of sharpness and dullness in the right mandibular nerve branches, decreased right masseter muscle contractions, and tenderness on the right GB 20 point. Electromyography revealed partial functional lesions in the postganglion at the right fifth cranial nerve. She had body acupuncture therapy at GV 20, GB 20, ST 5, ST 6, ST 7, CV 23, LI 4, HT 5, ST 36, LU 7, and KI 6; ear acupuncture at the Parotid and Shenmen points; and treatment with the Tan Balance Method. Results: After 3 consecutive sessions of acupuncture therapy, this patient's symptoms were reduced. Conclusions: Acupuncture was helpful for reducing paresthesia, dysarthria, xerostomia, dysgeusia, gum and lower right jaw pain, a chewing disorder, and cephalgia in this patient with postodontectomy neuropathy. Objective: Interventional pain management has been recognized over the last couple of decades for treating chronic-pain syndromes. Acupuncture is a nonpharmacologic therapeutic option for pain management and may be an option for different patients with contraindications for interventional pain management. This review explores this options. Method: This limited review examines the role of acupuncture for managing head-and-neck pain and lower-back pain, according to interventional pain management. Conclusions: Acupuncture at various points, corresponding to the stellate ganglion, which is ST 10 Shuitu, and corresponding to the splanchnic nerve and the facet joint of the lumbar vertebra - which are Ex-B2 paravertebra - can be applied for pain management in the head-and-neck area and in the lower-back area. Objective: Obesity, a condition with serious complications, needs special attention. It is a complex and multifactorial problem and regulation of calorie balance involving various humoral and central factors is the main key for managing obesity. In addition, there is an increase in various proinflammatory cytokines and an increase in oxidative stress. There is a need to discover a useful therapy for obesity management. The goal of this review was to examine the literature on electroacupuncture (EA) as a potential therapy. Methods: This review explores the literature on EA, which has proven to be effective for inducing weight loss in experimental human and animal studies. Both continuous and dense-disperse EA waves have their own roles in hormone regulation of obesity using ST 25, CV 9, CV 12, CV 4, SP 6, ST 36, and ST 44; this is discussed the associated mechanism related to this is through suppression of various orexigenic peptides, enhancement of anorexigenic peptides, suppression of inflammatory factors, and improvement in the balance of pro-oxidants and antioxidants. Conclusions: The absence of another definitive therapy for obesity and EA's minimal side-effects make it a potential therapy for managing obesity. © Copyright 2021, Mary Ann Liebert, Inc., publishers 2021. |

Background: Infertility affects ∼15%-20% of couples. Of the 16.7% infertility rate reported based on Word Health Organisation questionnaire data, 6.4% of cases are associated with male factors. Male infertility can result from abnormal semen parameters; oligoasthenoteratozoospermia (OAT) is the most common diagnosis. Acupuncture is an ancient method that has been used for centuries to treat and prevent various conditions. In modern medicine, it is gaining popularity as a complementary infertility therapy. Case: A 41-year-old male presented to the Medical Acupuncture Department of Dr. Cipto Mangunkusumo Hospital (in Jakarta, Indonesia) because he wanted to have a child. His semen was analyzed, and he was diagnosed with severe OAT. Manual acupuncture therapy was performed at CV 3, CV 4, CV 5, CV 6, CV 7, ST 29, SP 6, SP 3, ST 36, and KI 3, alternating with KI 7 5 days per week and at LR 8 once per week, for a total of 28 sessions. Results: Semen analysis after 2 series of manual acupuncture treatments revealed improvement, especially in sperm motility, from 25% to 33% and then to 67% after the first and second serial therapies, respectively, changing the diagnosis from severe OAT to severe oligoteratozoospermia. Conclusions: Manual acupuncture combined with medication improves male fertility, especially sperm motility. © Copyright 2021, Mary Ann Liebert, Inc., publishers 2021. |
