Introduction: The current ‘gold-standard’ treatment of critical-sized bone defects (CSBDs) is autografts; however, they have drawbacks including lack of massive bone source donor site morbidity, incomplete remodeling, and the risk of infection. One potential treatment for treating CSBDs is bone marrow-derived mesenchymal stem cells (BM-MSCs). Previously, there were no studies regarding the use of human umbilical cord-mesenchymal stem cells (hUC-MSCs) for treating BDs. We aim to investigate the use of allogeneic hUC-MSCs for treating CSBDs. Method: We included subjects who were diagnosed with non-union fracture with CSBDs who agreed to undergo hUC-MSCs implantation. All patients were given allogeneic hUC-MSCs. All MSCs were obtained and cultured using the multiple-harvest explant method. Subjects were evaluated functionally using the Lower Extremity Functional Scale (LEFS) and radiologically by volume defect reduction. Result: A total of seven (3 male, 4 female) subjects were recruited for this study. The subjects age ranged from 14 to 62 years. All seven subjects had increased LEFS during the end of the follow-up period, indicating improved functional ability. The follow-up period ranged from 12 to 36 months. One subject had wound dehiscence and infection, and two subjects developed partial union. Conclusion: Umbilical cord mesenchymal stem cells are a potential new treatment for CSBDs. Additional studies with larger samples and control groups are required to further investigate the safety and efficacy of umbilical cord-derived mesenchymal stem cells for treating CSBDs. © 2020, Springer-Verlag France SAS, part of Springer Nature. |

Background: Auricular cartilage is considered the source for cartilage grafts with good esthetic results and is often used in augmentation rhinoplasty. Most revision procedures require grafting, and the auricular cartilage can be the source for secondary rhinoplasty. Due to limited donor locations, donor site optimization of graft material is very important. We aimed to evaluate the healing process from the donor site, including the regeneration of cartilage from the donor site in an experimental animal model. Methods: Six New Zealand white rabbits were used for the investigation of cartilage regeneration from donor defects with one-sided perichondrium. Cartilage defect measuring 0.5 × 3 cm was made in rabbit ears. Two trial groups with 12 ears in each group were formed (one-sided perichondrium vs without perichondrium). Macroscopic and microscopic evaluation examinations were performed at week 4. The gap and thickness of the donor defect were measured with a centimeter grinder for macroscopic evaluation. Results: While macroscopic evaluation of wound thickness and the gap was not significant between groups (p = 1.00, p = 0.053, respectively), microscopic evaluation showed immature cartilage formation at week 4 of the donor defect with one-sided perichondrium (p = 0.004). Conclusions: The donor defect with one-sided perichondrium showed new cartilage formation within the area of the perichondrium. These findings showed the regeneration of cartilage was developed at the donor defects with one-sided perichondrium in the fourth week. OBJECTIVE: This study aims to construct a prediction model based on non-invasive examination and cardiovascular risk factors, to predict the presence of coronary artery disease (CAD) and its severity in patients with low-risk unstable angina pectoris (UAP)/ Non-ST Segment Elevation Myocardial Infarction (NSTEMI). PATIENTS AND METHODS: This cross-sectional study aimed to assess the association between non-invasive examinations and cardiovascular risk factors in predicting CAD. Model constructed based on non-invasive assessment and cardiovascular risk factors was compared to coronary angiography, the reference standard. RESULTS: This study included 104 patients, comprising 60 men and 44 women, who fulfilled the inclusion criteria. The mean age was 52.3 (6.8) years. Two diagnostic prediction models were constructed after series of analyses. The main model consists of NO, CIMT, history of smoking, and Age-Gender, while the alternative model consists of CIMT, history of smoking, and Age-Gender. The main model has AUC of 74.5% (95% CI: 64.9-84.1), sensitivity of 72.7% (95% CI: 57.2-85.0), specificity 65.0% (95% CI: 51.6 -76.9 for a cut-off point of 74.5. While the alternative model has 69.0% AUC (95% CI: 58.9-79.1), sensitivity of 65.9% (95%: 50.1-79, 5), a specificity of 56.7% (95% CI: 43.2-69.4) for a cut-off point of 69. The main model and the alternative model have similar diagnostic prediction performance based on the ROC comparison test (p = 0.70). CONCLUSIONS: Based on these results, we conclude that NO, CIMT, smoking history, and age-gender have a value of diagnostic validity in subjects with low-risk UAP/NSTEMI. © 2021 Verduci Editore s.r.l. OBJECTIVE: Several studies demonstrated that a high body mass index (BMI) might actually benefit patients with cardiovascular disease, including coronary heart disease. However, other studies were unable to confirm this paradoxical phenomenon in all populations. Therefore, this study aims to determine the association between BMI and long-term clinical outcomes in ST-segment elevation myocardial infarction (STEMI) patients undergoing percutaneous coronary intervention (PCI). PATIENTS AND METHODS: This was a retrospective cohort study of 400 STEMI patients undergoing PCI. Clinical outcome evaluation was done by face-to-face or phone interview and collecting objective data. Statistical analysis was performed to compare the outcomes between underweight-normal group with overweight- obese group. RESULTS: The incidence of major adverse cardiovascular events (MACE) was lower in patients with higher BMI group in 2-years evaluation (24.1% vs. 39.9%; p < 0.001). Multivariate analysis showed that BMI was an independent predictor of MACE and the incidence of recurrent infarction (OR 2.322 [CI 95% 1.505-3.584; p < 0.001]). The risk of MACE reduces as the weight increases, with a nadir of risk reduction for MACE at 28 to 29.0 kg/m2, in which the curve rises after, but remained below the risk associated with BMI of 23 kg/m2. CONCLUSIONS: In our population, patients with high BMI have a lower incidence of longterm MACE, especially recurrent myocardial infarction, in patients with STEMI undergoing PCI. © 2021 Verduci Editore s.r.l. Background: Cytokine storm has been widely known to contribute to the development of the critical condition in patients with coronavirus disease 2019 (COVID-19), and studies had been conducted to assess the potential aspect of cytokine storm elimination by performing therapeutic plasma exchange (TPE). However, contradictory findings were observed. The objective of this study was to assess the association between TPE and the reduction of mortality of critically ill COVID-19 patients. Methods: A meta-analysis was conducted by collecting data from PubMed, Scopus, and Web of Science. Data on the mortality rate of critically ill COVID-19 patients treated with TPE plus standard of care and that of patients treated with standard of care alone were analyzed using a Z test. Results: We included a total of four papers assessing the association between TPE and the risk of mortality among critically ill COVID-19 patients. Our findings suggested that critically ill COVID-19 patients treated with TPE had lower risk of mortality compared to those without TPE treatment. Background: Subgingival niche is one biofilm habitat containing rich microbiota, which plays an active role in maintaining the health of periodontal tissue and determining host response. As such, a study of changing subgingival biofilms is important for understanding the effect of a systemic condition. In this study, we compared the occurrence of six bacteria cohabiting in the subgingival area of periodontitis subjects, with (DP, n = 8) and without (NDP, n = 4) diabetes. Methods: The six genus and species of targeted bacteria were confirmed by 16S rRNA amplicon sequencing on MinION nanopore platform. Descriptive statistic was used to describe the obtained data. Results: We found that the six genus and species of targeted bacteria were detected but in different quantities in either group's periodontal pocket. Our data showed that Tannerella forsythia was the most abundant species in subgingival biofilms of the DP group of the red complex bacteria. In contrast, Aggregatibacter sp., which belongs to the phylum of proteobacteria, was present at a relatively lower level. In contrast, Fusobacterium sp., which belongs to orange complex bacteria, showed relative similarities in subgingival biofilms of both groups tested, while Veillonella sp., were abundant in the DP groups. Conclusions: Our data show that the diversity of classic periodontopathogens increased in the subgingival niche of periodontitis subjects with diabetes. It is the first study in Indonesia to apply MinION-based, full-length 16S rRNA amplicon sequencing in periodontitis patients with and without diabetes. © 2021. Bachtiar BM et al. |

Background: Persistent immune activation and inflammation in HIVinfection are linked to excess cardiovascular risk and other noncommunicable diseases. Periodic asymptomatic CMV-reactivity in HIV infected patients over a lifetime may contribute to non-AIDS defining morbidity. Despite undetectable levels of HIV and CMV, these patients continue to have increased levels of biomarkers and immune activations. Statin administration is thought to reduce subclinical atherosclerosis by decreasing LDL-C levels. It may also add beneficial effects against CMV infection. Methods: We are conducting a double-blind placebo-controlled trial in which patients are randomized to receive either atorvastatin or placebo with a ratio of 1:1. This trial aims to study the effect of atorvastatin in statin-naive virally-suppressed HIV-infected patients with stable ART and CMV seropositivity on carotid intima media thickness (CIMT), tool that evaluates subclinical atherosclerosis. The study recruits 80 patients at HIV integrated care unit of Cipto Mangunkusumo hospital. All eligible subjects have CIMT evaluation as primary outcome, along with flow mediated vasodilatation (FMD), liver fibrosis and steatosis evaluation, fasting lipid, neurocognitive test, community periodontal index (CPI), and residual immune activation as secondary outcomes in 48 weeks. Ethics and dissemination: This study has received an ethical approval from Health Research Ethics Commitee–Universitas Indonesia and Cipto Mangunkusumo Hospital. Before joining the study, all participants fill in an informed consent form. At the end of study analysis, the trial results will be published and disseminated in peerreviewed journals. Background: Research in natural substances for their anticancer potential has become increasingly popular. Lunasin, a soybean protein, is known to inhibit cancer progression via various pathways. The aim of this study was to investigate the effect of Lunasin Extract (LE) on the expression of Intercellular Adhesion Molecule 1 (ICAM-1) and epithelial cadherins (E-Cadherin) in breast cancer. Methods: In this true-experimental in vivo study, 24 Sprague-Dawley rats that were induced by 7,12-Dimethylbenz[a]anthracene (DMBA), were used. Based on the therapy given, the groups were divided into, normal, positive control (PC), negative control (NC), adjuvant, curative, and preventive. Lunasin was extracted from soybean seeds of the Grobogan variety in Indonesia. Tissue samples were obtained, processed, stained with anti-ICAM-1 and anti-E-Cadherin antibodies, examined under a microscope, and quantified using H-score. The data were analyzed using ANOVA, which was then followed by Duncan's test. Results: Statistically significant difference in ICAM-1 expression was observed between the following groups: adjuvant and NC, normal and NC, PC and NC, adjuvant and preventive, normal and preventive, PC and preventive, adjuvant and curative, normal and curative, PC and curative. E-Cadherin expression was significantly different between preventive and NC, adjuvant and NC, PC and NC, normal and NC, adjuvant and curative, PC and curative, normal and curative, normal and preventive. Significant negative correlation was found between ICAM-1 and E-Cadherin [-0.616 (-0.8165; -0.283)] with p = 0.001.
