Radiotherapy is one of the treatments for nasopharyngeal cancer (NPC). However, this treatment might produce an unfavorable effect on the thyroid gland, which eventually results in less production of thyroid hormone. This is condition is known as hypothyroidism. The development of hypothyroidism in each patient with post-radiative NPC differs according to several factors. This study aims to analyze the rate of development of hypothyroidism in post-radiated NPC patients. This aim is achieved by identifying subgroups of patients with different hazard rates of developing hypothyroidism, and further identify factors explaining hypothyroidism in each subgroup. Data on ninety-seven NPC post-radiation patients taken from one of the hospitals in Jakarta were analyzed. Survival tree with the relative risk tree algorithm was proposed to analyze the data. We identified three subgroups of patients with relatively slow, medium, and fast developing of hypothyroidism. For the slow subgroup, 26% of the patients developed hypothyroidism at 150+ weeks post-radiation, while it only took less than 30 weeks for those in fast-growing subgroup; and 70 until 130 weeks for the medium subgroup. We also found that sweat production and Zulewski's total score were the important factors in explaining the development rate of hypothyroidism. © 2021 Institute of Physics Publishing. BACKGROUND Hirschsprung-associated enterocolitis (HAEC) is a life-threatening complication of Hirschsprung’s disease. Studies using animal models on the pathogenesis of HAEC are limited. Thus, this study aimed to establish a rat model of HAEC using topical application of 0.1% benzalkonium chloride (BAC) in the sigmoid colon. METHODS 55 male Sprague Dawley rats aged 10−12 weeks old were separated into 11 groups. The control group (n = 5) was euthanized on day-7, and the other 10 groups (n = 5 in each group) treated with 0.1% BAC in the sigmoid colon for 15 min to induce Hirschsprung’s disease were euthanized on day-7,-10,-12,-14,-17,-19,-21,-23,-25, and-28. The sigmoid colon was excised, fixed in formalin, and sectioned for histological examinations with hematoxylin and eosin staining. The degree of HAEC was compared within all groups. RESULTS Rats that were sacrificed on day-7 to-12 showed the 1st degree or early HAEC, which was most likely caused by BAC application. The 2nd degree of HAEC occurred in rats that were sacrificed on day-14 that showed a macrophage infiltration in the sigmoid colon, thus fulfilled the initial criteria for HAEC (p = 0.0025 versus control). The degree of enterocolitis increased with time, and the highest degree was found in rats that were sacrificed on day-28 (p<0.001 versus control). CONCLUSIONS Topical application of 0.1% BAC for 15 min was successfully produced HAEC model in rats, which was occurred on day-14 after the application. This model provides a useful resource for further research on the pathogenesis of HAEC. © 2021 Authors. |

BACKGROUND Some patients who undergo pelvic floor reconstruction for pelvic organ prolapse (POP) may experience a de novo stress urinary incontinence (SUI) postoperatively. We aimed to investigate the incidence and characteristics of de novo SUI in patients who underwent pelvic floor reconstruction at the national referral hospital in Indonesia. METHODS This cross-sectional study evaluated 108 patients who underwent pelvic floor reconstruction due to POP between January 2016 and December 2017. Per the inclusion criteria, 75 women were enrolled using a consecutive sampling. The incidence of de novo SUI was determined 6–12 months postoperatively using the Indonesian version of the questionnaire for urinary incontinence diagnosis and objectively using the cough stress test during gynecologic examination after a negative preoperative prolapse reduction stress test. RESULTS The average age, parity, body mass index, and years since menopause onset were 56.17 (4.67) years, 3.17 (1.07), 28.58 (5.18) kg/m2, and 12.8 (7.0) years, respectively. De novo SUI was seen in 8.0% (6 of 75) patients at 6–7 months postoperatively, with 3 (50.0%) had severe POP and 3 (50.0%) had a mild POP. Most of these patients (4 of 6, 66.7%) had undergone procedures other than colpocleisis for POP reconstruction. CONCLUSIONS The incidence of de novo SUI after gynecologic surgery for POP at a national referral hospital in Indonesia is 8%. Most patients were aged <60 years, had a parity of <4, were nonobese, were menopausal, and had diabetes. © 2021 Authors. |

Background. Brain tumor is a rare tumor with low incidence. Although it is a rare tumor, the mortality of brain tumor is disproportionately high. Many countries have already published epidemiology of brain tumor. However, the epidemiology of brain tumor in Indonesia remains to be investigated. This article aimed to provide descriptive epidemiology of brain tumor. Methods. The data of brain tumor acquired from Department of Neurology and Neurosurgery Cipto Mangunkusumo General Hospital from 2014 to 2016. The histopathology classification of primary intracranial tumors was based on WHO classification of CNS tumors 2016 while brain metastasis was classified based on other histopathological types. The vari-ables were analyzed and presented descriptively. Results. There were 316 subjects acquired in this study. Most of the subjects (68%) were women. The mean age of this study was 43.8 (11.7). Most of the subjects (86.1%) had primary histopathology. Lung cancer was the most commonly found brain metastasis in this study (34.1%). Conclusions. This is a pilot study of brain tumor epidemiology in Indonesia. Collaboration with other centers in Indonesia is needed to give a more representative insight of brain tumor in Indonesia. © 2021, Editura Medicala. All rights reserved. |

BACKGROUND: Cancer stem cells (CSCs) is defined as tumor initiating cells within tumor that maintain stemness properties and tumorigenicity. Extracellular pH of CSCs in in vitro condition is important for supporting cell proliferation which may also regulate the expression of stemness markers such as OCT4. This work aimed to examine the effect of cell culture media on the proliferation and stemness of human breast cancer stem cells (BCSCs). RESULTS: Our results showed a higher viable cell number and proliferation of BCSCs in DMEM/F-12 HEPES (-) compared to HEPES (+) medium until 4 day incubation. OCT4 mRNA and protein level, as well as MFU of BCSCs were significantly higher in HEPES (-) compared to HEPES (+) medium on day 2. CONCLUSION: DMEM/F-12 medium without HEPES facilitates CD24-/CD44+ BCSCs to have higher proliferation and stemness on day 2 incubation compared to those with HEPES. © 2021. All Rights Reserved. |

Pulmonary alveolar proteinosis (PAP) is a rare disease with mostly due to autoimmune toward granulocyte-macropahge colony-stimulating factor. In some conditions, PAP followed with secondary infection. A 34-year-old woman came with progressive shortness of breath, chronic dry cough, and mild fever. The chest High-Resolution Computed Tomography showed ground-glass opacity with septal reticulation or known as the crazy-paving pattern, and a cavity on the upper lobe of the left lung. The patient underwent bronchoscopy for diagnostic and therapeutic measures and found milky appearance bronchoalveolar lavage fluid (BALF). The serum galactomannan came out positive. Fungal infection detected from the BALF culture, Aspergillus fumigatus, hence fulfilling the diagnosis of PAP with probable invasive pulmonary aspergillosis. Colorectal cancer (CRC) is the world's third most cancer and the second highest mortality rate. The searching for new anti-inflammation substances with less adverse effects than aspirin for chemoprevention and adjuvant chemotherapy of CRC is running. The most notable one is fish oil containing omega 3. Kusmardi, et al. studied that industrial waste fish oil omega-3 level comes close enough to conventional fish oil industry. Study aims to reducing the level IL-6 on mice colon tissue being induced CRC using AOM/DSS by fish oil administration. Thirty male Swiss Webster mice are grouped into six treatments: Positive control (aspirin), negative control (physiological saline), normal, high dose (fish oil 6mg/kgBW), medium dose (fish oil 3mg/kgBW), dan solvent control (corn oil). Colon tissue was stained using anti IL-6 antibody. Ten photos per slide were taken by microscope (400x), analyzed for the IL-6 expression by ImageJ®, and quantified for H-score. Data was analyzed using SPSS 24.0 (CI 95%) and p-value <0.05 is consider significant. Data are not normally distributed with median of 161.64 (119.4-260.67). Kruskal-Wallis test is significant in addition with Mann-Whitney test shows only high dose group has significant difference to negative control (p=0.008), medium dose (p=0.016) dan and solvent control (p=0.008). No significant difference reported between high dose and positive control group (p=0.69). High dose industrial waste fish oil can lower IL-6 expression in mice colon tissue induced CRC using AOM/DSS. © 2021 Oriental Scientific Publishing Company. |

Background: Primary breast lymphoma is a rare disease and accounts for 0.4–0.5% of malignant breast neoplasms and 1.7–2.2% of extra-nodal lymphomas, with diffuse large B-cell lymphoma (DLBCL) as the most common histologic subtype. Case: A 47-year-old female with beta thalassemia presented with a lump of the left breast, redness, pain, and swelling of her left breast. Physical examination showed tender, red, swollen left breast. Laboratory findings show mild anemia and normal level of lactate dehydrogenase 329 U/L (normal range: 240–480 U/L). PET scan showed hypermetabolic mass with irregular margins covering the whole left breast quadrants with the size of 11.25 × 5.17 cm with left pectoralis major, left parasternal, and left axillary hypermetabolic nodules. Histopathology and immunohistochemistry staining showed a non-germinal center B-cell-like subtype of DLBCL CD20+. We administered the R-CHOP regi-men (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednison) every 3 weeks for 6 cycles. The response was complete remission. The patient tolerated the chemotherapy well and achieved long term complete remission. Conclusion: Primary breast lymphoma is a rare disease with the most common subtype is diffuse large B-cell lymphoma. Myxoma shows unspecific symptoms similar to other cardiac disorders. Late resection of myxoma will worsen output such as congestive heart failure, cardiac arrhythmia, and cardiac embolism, causing sudden death. Early diagnosis and surgical excision of cardiac myxomas, especially with the villous surface, must be done as soon as possible. Additional diagnostic examination was needed to define types of myxoma for earlier treatment to prevent complications. We present a case of atrial myxoma which was diagnosed and treated earlier than the median time reviewed by the previous study. This paper focuses on the importance of early diagnosis and defining types of myxoma. © 2021, ÄŒKS. |
