Background: Interstitial cystitis (IC) is a chronic inflammation of the bladder which causes unpleasant sensations. This condition considerably reduces patients’ quality of life. Intravesical hyaluronic acid is one of the new interventions for interstitial cystitis management, in which a thorough review and meta-analysis regarding this topic are still limited. This study aimed to compare the efficacy of intravesical hyaluronic acid with other intravesical treatment modalities available to treat interstitial cystitis. Method: Systematic literature searching with the main keywords of “interstitial cystitis” or “bladder pain syndrome” and “intravesical hyaluronic acid” was conducted. The main outcome of this study was pain assessment using a visual analogue scale (VAS) and Interstitial Cystitis Symptoms Index (ICSI), and problem index (ICPI). Meta-analysis was carried out when comparative studies were available. Eleven studies were obtained to assess intravesical hyaluronic acid’s effects, including three in the meta-analysis. The intravesical treatments found as a comparison were heparin and chondroitin sulphate. Result: There was significant heterogeneity among studies included in all parameters. The pooled analysis showed no significant difference in VAS reduction, improvement of ICSI and ICPI score (p = 0.11, p = 0.35, and p = 0.28, respectively) between hyaluronic acid and other intravesical treatments. Conclusion: There was no significant difference between hyaluronic acid and other intravesical treatments. However, further research with a larger sample is needed to confirm the best modalities in interstitial cystitis. © 2021, Sanglah General Hospital. All rights reserved. |

Introduction: Chronic Plantar Ulcer of Leprosy (CPUL), one of the debilitating disabilities and complications, poses a treatment challenge. Utilization of stem cells, specifically adipocyte-derived mesenchymal stem cells-conditioned medium (ADMSC-CM) and amniotic membrane mesenchymal stem cells-conditioned medium (AMSC-CM), may serve as a novel therapeutic option. This study aims to compare the efficacy of topical ADMSC-CM, topical AMSC-CM, and farmazertin gauze dressing (FGD) only on the healing of CPUL. Methods: In this randomized controlled trial, our study recruited 54 subjects with CPUL. Subjects were randomly assigned to topical ADMSC-CM (n = 16), topical AMSC-CM (n = 22), or FGD (n = 16) applied every three days for up to 8 weeks. Data were analyzed using SPSS version 20 for Windows. Results: Healing percentage increased each week in all groups. Statistic differences between groups (p <0.05) were observed for ulcer mean size and depth reduction from week three onwards. No adverse reactions or complications were reported. At the end of the study, based on clinically improved ulcers, topical ADMSC-CM (100.0%) were superior in improving the healing of CPULs compared to topical AMSC-CM (54.55%) and FGD only (50.0%). Conclusion: ADMSC-CM and AMSC-CM are potential therapeutic agents in the management of CPUL, with ADMSC-CM being superior in overall ulcer improvement. © 2021, Sanglah General Hospital. Since coronavirus disease 2019 was declared a global pandemic by the World Health Organization, it has become a challenging situation to continue medical education, including in Indonesia. The situation prohibited face-to-face (direct) educational activities in clinical settings, therefore also postponing examinations involving especially procedural skills. Adaptations were urgently needed to maintain the delivery of high-stake examinations to sustain the number of ophthalmology graduates and the continuation of eye health service. Objective structured clinical examination (OSCE) has been one of our widely used method to assess clinical competencies for ophthalmology residents, and is the one method that involves gatherings, close contact of examiners, examinees and patients, therefore the most difficult to adjust. Pandemic challenges brought technical changes in our delivering the OSCE to online, maximizing digital platforms of meetings, while still concerned to guarding the safety of candidates, patients and staffs. OSCE scenarios were also made as timely efficient as possible by changing continuous station models to a cascade one. The purpose of this article is to document our experience in conducting a feasible and reproducible OSCE in this pandemic era filled with limitations. © The Korean Society of Medical Education. Purpose: To assess knowledge retention of physicians after participating in a webinar series and its perceived benefits on daily practice and career development. Methods: The webinar series comprised six separate webinars about daily practice. Online questionnaires were sent to all participating physicians via email 3 months after the webinars. The questionnaire assessed knowledge retention through the difference between initial and follow-up post-test as well as the webinar series' benefits on daily practice and career development. Participants' demographic information, including their age, gender, education, year of graduation, and work details, were collected to compare outcomes between demographic groups. Results: A total of 689 responses were gathered, and 622 were analyzed. At follow-up, the median knowledge score was significantly lower than the initial median knowledge score (Z=-6.973, p<0.001). Participants' perception of the webinar series' benefits on daily practice and career development was very positive. A significant weak negative correlation was found between age and knowledge score at follow-up (rs=-0.157, p<0.001). Physicians who graduated more recently and worked for less than 3 years scored significantly higher on knowledge tests at follow-up. Meanwhile, perception score towards webinar series' benefits on daily practice was significantly higher among physicians with more extended work history. Male physicians scored significantly higher on perception score towards webinar series' benefits on career development. Conclusion: Online continuing medical education programs like webinars can encourage physicians to maintain their competence, but further research on improving knowledge retention over time is necessary. Overall, physicians perceived webinars to be beneficial for their professional development. © The Korean Society of Medical Education. All rights reserved. |

Background: Treatment for sex hormone depletion along with chronic inflammation would be beneficial for aging males. Mesenchymal stem cells (MSC) are known for their immunomodulatory activities and differentiation ability in regenerative medicine. Whether MSC could influence both testosterone and pro-inflammatory marker in aging males is uncertain. This study aimed to explore the effects of human umbilical mesenchymal stem cell (hUCMSC) on testosterone, tumor necrosis factor-alpha (TNF-α), and creatinine levels in aged rats. Methods: The hUCMSC was administered to aged male Sprague-Dawley rats (24 months old). After four injections of 1 mil-lion per kg body weight in 3-month intervals, the rats were sacrificed, and serum was collected for biochemical examinations. Results: The hUCMSC administrations increased the testosterone level almost three-fold and decreased the TNF-α level. Moreover, the high testosterone level was strongly correlated with low TNF-α level (p = 0.013; r =-0.863) in aged male rats. These results were following our previous report, which showed that the hUCMSC increased the number of Leydig cells. Serum creatinine levels in the treatment group were slightly increased but were still within the normal limit. Conclusion: The hUCMSC treatment in aged male rats tends to increase testosterone levels and lower TNF-α levels. © 2021 Japan University of Health Sciences & Japan International Cultural Exchange Foundation. |

Background: The incidence of sexual dysfunction increases in women with pelvic organ prolapse. In addition to physical factors, other important components that influence each other in sexual dysfunction are psychological aspects and genital self-image. Sociocultural factors also affect individual sexuality and sexual behavior. Until now, there are no data and the relationship between genital self-image and sexual dysfunction in pelvic organ prolapse is not known in Indonesia. Objective: This study aims to analyze the correlation of genital self-image with sexual dysfunction in women with pelvic organ prolapse. Methods: In this cross-sectional study, 113 consecutive women with pelvic organ prolapse were selected in urogynecology referral centers in Jakarta. Demographic data, physical examination, and guided interviews were filled in to complete the Female Sexual Function Index and Female Genital Self-Image Scale-7 questionnaires. Preliminary research has been carried out in the form of language translation and cultural validation of the Indonesian version of the Female Genital Self-Image Scale-7 questionnaires. Results: There is a significant correlation between genital self-image and sexual dysfunction, where the lower Female Genital Self-Image Scale-7 score significantly predicts the lower Female Sexual Function Index score (p < 0.000; odds ratio: 14.17). Conclusion: Genital self-image is the main variable that affects sexual function in women with pelvic organ prolapse. It is necessary to evaluate and treat genital self-image because sexual function is an integrated component of quality of life. © The Author(s) 2021. |

Background: Inflammatory bowel disease is a chronic inflammation caused by the malignant inflammation response and if not treated, could lead to colorectal cancer. One of the researched treatment is mahkota dewa (Phaleria macrocarpa) leaf extract that has flavonoid compound known to reduce inflammation. This study was aimed to prove that mahkota dewa leaf extract could reduce inflammation of mice colon induced with dextran sodium sulfate (DSS) and observe MUC1 expression from colon epithelial crypt of Lieberkuhn. Methods: This was a laboratory experiment using biological material (paraffin block) taken from 28 mice and divided into 5 groups: normal, aspirin, low and high dose mahkota dewa, and negative control. They were processed into immunohistochemistry and stained microscopic slides. Afterwards, they were observed with 400x magnification and 5 field-of-view of mice colon crypt of lieberkuhn. Then MUC1 expression was counted using ImageJ to obtain mean immunohistochemistry score and analyzed with SPSS. Results: There were significant reduction of MUC1 expressions from normal, aspirin, and high dose mahkota dewa groups compared to the negative control group. The result shown MUC1 expression from high dose mahkota dewa (M=149.90,SD=3.81) and aspirin (M=158.92,SD=5.28) were closer to normal group (M=148.02,SD=5.28). There were no significant results between negative (M=175.39,SD=14.30) and low dose mahkota dewa group (M=149.90,SD=5.02). Conclusion: There was a reduction of MUC1 expression in DSS-induced mice colonic epithelial cells for high dose mahkota dewa group. Background: Lunasin Targeted Extract (ET-Lun) has a pharmacology effect in inhibiting inflammation by decreasing COX-2 and iNOS expression. ET-Lun could increase apoptosis and decrease dysplasia (p > 0,05). In addition, ET-Lun could decrease EGFR expression in breast cancer rats. The acute toxicity showed ET-Lun has LD50 more than 5000 mg/kg BW and was practically non-toxic. Objective: this study aimed to determine the subchronic toxicity of ET-Lun. Methods: Male and female Sprague Dawley rats (n=40) were divided into 4 groups, the control group and treatment group ET-Lun dose of 250 mg/Kg BW, 500 mg/kg BW, and 750 mg/kg BW. The ET-Lun was administered for 90 days. On the 91st day, the animals were dissected and examined for SGOT-SGPT levels, liver histopathology, and diameter of the central vein. Results: The SGOT-SGPT levels showed no significant difference between the treatment group and the control group (p > 0.05). On microscopic observation, there was no change or damage to the liver of rats in each group. The diameter of the central vein of the rat liver shows no significant difference between the control and treatment groups. Conclusion: The ET-Lun does not produce adverse effects in liver rats after subchronic treatment. © 2021 Phcogj.Com. |

Malaria still be health problem in the world, especially in Eastern Indonesia. Malaria’s inflammation and metabolism defect can cause colonic damage, such as enhancement Muc-1 protein expression and goblet cells hyperplasia. Sambiloto and spirulina combination as antiinflammatory and antioxidative agent can prevent medial colon damage Plasmodium berghei ANKA infected mice. The aim of the study to show the effect of sambiloto and spirulina combination on Muc-1 protein activity in medial colon Plasmodium berghei ANKA infected mice. This study use preserve male Swiss Webser mice colonic tissue which has inoculated by Plasmodium berghei ANKA, whose treatment group include positive control (dehyroartemisin piperaquine), negative control (carboxymethil cellulose), AP (sambiloto), AP+ES (sambiloto+spirulina extract), and AP+PS (sambiloto+spirulina powder) and terminated after 28 days of treatment. Colonic tissue was stained with immunohistochemistry and observed using light microscope (400x) in five different field and was analyzed with ImageJ® sowtware, and statisitcal analysis was done with SPSS 20.0. According to One Way ANOVA and Duncan posthoc test, only AP+PS(120,98 ±3,37), which significantly difference between negative control, AP, and AP+ES group. Meanwhile, between DHP, AP+PS group not significantly differenece. Sambiloto extract and spirulina powder combination can reduce Muc-1 protein expression in medial colon Plasmodium berghei ANKA infected mice. © 2021 Phcogj.Com. |

Introduction: Therapeutic plasma exchange (TPE) for neuroimmunological disorders has played an increasingly important role within the Southeast Asian (SEA) region. The South East Asian Therapeutic Plasma exchange Consortium (SEATPEC) was formed in 2018 to promote education and research on TPE within the region. The advent of the Covid-19 pandemic has produced challenges for the development and expansion of this service. Methodology: A qualitative and semi-quantitative questionnaire-based survey was conducted by SEATPEC member countries from January to June 2020 (Phase 1) and then from July 2020 to January 2021 in (Phase 2) to assess the impact of Covid-19 on regional TPE. Objectives: The study's main objectives were to explore the challenges experienced and adaptations/adjustments taken by SEATPEC countries in order to continue safe and efficient TPE during the Covid-19 pandemic. Results: The pandemic was found to disrupt the delivery of TPE services in all SEATPEC countries. Contributing factors were multifactorial due to overstretched medical services, staff shortages, quarantines and redeployments, fear of acquiring Covid-19, movement restriction orders, and patient's psychological fear of attending hospitals/testing for Covid-19. All SEATPEC countries practiced careful stratification of cases for TPE (electives vs emergencies, Covid-19 vs non-Covid-19 cases). SEATPEC countries had to modify TPE treatment protocols to include careful preprocedure screening of patient's for Covid-19, use of personal protective equipment (PPE) and post-TPE sanitization of machines and TPE suites. Conclusion: Based on the responses of the survey, SEATPEC countries produced a consensus statement with five recommendations for safe and effective TPE within the region. © 2021 Wiley Periodicals LLC. |
