Purpose: To assess the clinical relevance of pathophysiology-based biomarkers, specifically serum C1q and whole blood interferon gene signature score (IGSS), in ocular tuberculosis (OTB) diagnosis by conducting an integrative analysis of clinical presentations and treatment response. Methods: This retrospective cohort study analysed data from 70 patients with suspected OTB at a tertiary care uveitis practice in Indonesia. Serum C1q levels and whole blood IGSS were quantified. Patients were categorized into four quadrants based on their biomarker profiles: quadrant 1 (high C1q & low IGSS), quadrant 2 (high C1q & high IGSS), quadrant 3 (low C1q & high IGSS), and quadrant 4 (low C1q & low IGSS). Characteristics of clinical presentations, work-up results, and treatment outcomes were explored according to the predefined quadrants. Results: We identified that the majority of OTB patients diagnosed with concurrent active pulmonary TB were in quadrant 1, 2, or 3 (20/23, 87.0%). Twenty-seven patients (27/47, 57.4%) with clinically undifferentiated uveitis were in quadrant 4 (p < 0.001). Among patients in quadrants 1, 2, and 3, completion of a full course of antitubercular treatment (ATT) was associated with a lower number of patients showing persistence or recurrence of ocular inflammation compared to those who were not fully treated with ATT (14.3% vs 85.7%, p = 0.001). Conclusions: Based on the analysis of clinical features and treatment outcomes, patients with elevated levels of either or both serum C1q and whole blood IGSS may reflect active TB disease in the eye, necessitating full ATT management. © 2024 Taylor & Francis Group, LLC. |

Vesicoureteral reflux (VUR) is characterized by the backward flow of urine from the bladder to the upper urinary tract, causing kidney damage if left untreated. However, the current diagnostic method, voiding cystourethrography (VCUG), is invasive, potentially risky, and may cause anxiety, especially for young patients and their caregivers. Urinary extracellular vesicles (uEVs) are small vesicles originating from the renal tubular system that carry lipids, proteins, and RNAs for intercellular communication and regulation. uEVs have emerged as promising human biofluid-derived biomarkers in various kidney diseases. This chapter explores the potential of uEVs as noninvasive biomarkers for diagnosing VUR, monitoring its progression, and determining the extent of kidney damage. Previously, serum inflammatory markers have been identified as factors associated with kidney fibrosis in reflux nephropathy, but these may simply be a reflection of systemic inflammation rather than of a specific condition. Meanwhile, current research on uEVs in VUR remains limited. One study has identified vitronectin, a uEV-derived protein, as a predictor of VUR induced by spinal cord injury (sensitivity = 80%, specificity = 82.9%, area under the curve (AUC) = 0.795). Another study, focusing on pediatric VUR, showed that patients with kidney fibrosis had significantly higher urinary neutrophil gelatinaseassociated lipocalin (uNGAL) values than those without kidney fibrosis (1.49 ng/mL vs. 0.58 ng/mL, p < 0.001). However, apart from kidney fibrosis, uNGAL also increases in the contexts of AKI and pediatric urinary tract infections, indicating that uNGAL is not specific as a single marker for VUR. These findings highlight the importance of identifying biomarkers derived from entities, such as uEVs, that are specific to the urinary system and are not affected by systemic factors In summary, uEVs offer crucial insights into urinary tract processes and have broad potential for diagnosing, treating, and forecasting kidney and urinary tract diseases, including VUR. Future studies to test their role should encompass larger sample sizes, standardized protocols, and rigorous validation. © 2024 Nova Science Publishers, Inc. All rights reserved. |

Lack of public awareness of spondyloarthritis (SpA) leads to diagnosis delay and high disease activity. This study aimed to assess the public knowledge, perceptions, and attitudes towards SpA in Indonesia. This research was a cross-sectional study with an online questionnaire. A total of 1084 participants’ demographics, knowledge, attitude, and perception of SpA were obtained using convenience sampling. Respondents’ knowledge, attitude, and perception levels were scored numerically and categorized (satisfactory and unsatisfactory). Associations were assessed using a logistic regression analysis. The mean ± standard deviation (SD) knowledge score was 5.21 ± 1.89 (45.11% of respondents had satisfactory knowledge). Living on Java Island was associated with the participants’ unsatisfactory knowledge (Adjusted Odds Ratio (AOR): 1.33; 95% confidence interval (CI): 1.03 to 1.71; p = 0.027). The mean ± SD perception score was 3.14 ± 1.21 (35.98% of respondents had a satisfactory perception). Education (bachelor AOR: 1.46; 95% CI 1.08 to 1.98; p = 0.014) and occupation (employed AOR: 1.53; 95% CI: 1.07 to 2.20; p = 0.020) were significantly associated with poor perception. The mean ± SD score of attitudes was 2.52 ± 1.54, while only 54.24% of participants preferred went to the doctor if they had symptoms of SpA. Employed respondents (AOR: 1.40; 95% CI: 1.07 to 2.01; p = 0.046) and living inside Java Island (AOR: 1.45; 95% CI: 1.13 to 1.87; p = 0.004) were associated with attitudes towards SpA. In conclusion, more than half of the subjects had a poor knowledge, perception, or attitude towards SpA. Therefore, programs that empower the knowledge, attitude, and perception towards SpA in community need to be developed to close this gap. © 2024 Institute of Health Promotion and Education. |

Objectives: Penile carcinoma (PC) is a rare disease with considerable physical and psychological impact. To date, there is no data regarding PC prevalence and characteristics in Indonesia. This study aimed to analyze the characteristics of patients with PC in Indonesia and determine cumulative survival rates and time to disease progression. Methods: This was a retrospective study of all patients diagnosed with PC at Cipto Mangunkusumo General Hospital from 1995 to 2014, with a minimum of 1 year follow-up. The outcomes of the study were cumulative survival rates and time-to-disease progression. Results: Ninety-three subjects were recruited, with a mean age of 49.44 ± 13.62. Inguinal lymph node dissection (ILND) was performed in 49 (53%) patients. The mean survival in the ILND group was better compared to the non-ILND group (80.7 months vs. 67.1 months; p = 0.032). Time-to-progression in the ILND group was significantly longer than in the non-ILND group (71.7 months vs. 54.3 months; p = 0.022). No significant difference in survival between the total and partial penectomy (PP) groups was observed (p = 0.701). Time-to-progression in total penectomy (TP) was significantly longer than in PP (68 months vs. 56.0 months; p = 0.023). In Cox-regression analysis, after adjustment of other variables, history of ILND, higher stage of cancer, and older age were found to affect the survival of patients. Conclusion: ILND in PC led to better survival and reduced disease progression. The type of penectomy is only associated with progression but not survival. TP had a longer time to disease progression compared to PP. © 2024 The Japanese Urological Association. |

Safety is of critical importance to chronic total occlusion (CTO) percutaneous coronary intervention (PCI). This global consensus statement provides guidance on how to optimise the safety of CTO) PCI, addressing the following 12 areas: 1. Set-up for safe CTO PCI; 2. Guide catheter-–associated vessel injuries; 3. Hydraulic dissection, extraplaque haematoma expansion, and aortic dissections; 4. Haemodynamic collapse during CTO PCI; 5. Side branch occlusion; 6. Perforations; 7. Equipment entrapment; 8. Vascular access considerations; 9. Contrast-induced acute kidney injury; 10. Radiation injury; 11 When to stop; and, 12. Proctorship. Introduction: This study aimed to assess the efficacy of adipose-derived mesenchymal stem cell exosomes (ASCs exosome) and platelet-rich fibrin (PRF) in treating critical long bone defects in Sprague-Dawley rats. Critical long bone defects, defined as exceeding 2 cm or 50% of the bone diameter, often pose a healing challenge. While autologous bone grafts have been considered, they have shown unreliable results and donor-site complications, necessitating alternative treatments. Methods: The research followed a quasi-experimental post-test only control group design involving 30 male Sprague-Dawley rats. The rats were divided into five groups and subjected to femur bone defect creation, internally fixed with a 1.4 mm K-wire, and treated with various combinations of hydroxyapatite (HA), bone graft (BG), ASCs exosome, and PRF. Histomorphometry and BMP-2 gene expression analysis were performed to evaluate bone healing. Results and Discussion: The results indicated that the group treated with HA + BG + ASCs exosome (group IV) exhibited the highest BMP-2 gene expression, while group III (HA + BG + ASCs exosome + PRF) had the highest chordin level. Overall, groups receiving ASCs exosome or PRF intervention showed elevated BMP-2 expression compared to the control group. The use of ASCs exosome and PRF showed comparable outcomes compared to bone graft administration in terms of histomorphometry analysis. Conclusion: The administration of adipose tissue derived mesenchymal stem cells and PRF has a comparable outcome with the use of bone graft in terms of osseus area and expression of BMP-2 in critical bone defect. © The Author(s) 2024. |

Study Design: Systematic review, expert opinion and Delphi technique, and validity and reliability studies. Objective: We developed Jakarta Instability Score (JIS) to identify spinal instability and the need of fusion in degenerative lumbar spinal stenosis (LSS). Methods: This study consisted of systematic review to find predictors of spinal instability, expert opinion and modified Delphi technique to develop JIS, and validity and reliability studies of the newly developed JIS. Results: A total of 54 studies were included in the systematic reviews to obtain predictors of spinal instability. Through expert opinion and modified Delphi technique, JIS was developed and consisted of the clinical component (back pain), dynamic radiograph component (dynamic translation and angulation), and MRI component (facet joint effusion), each of the component would be scored, and the total scoring would be from 0 to 14. The final scoring would classify patients into three groups: stable group (score of 0 to 4) in which the fusion is not needed, potentially unstable group (score of 5 to 8) in which the decision of fusion is based on surgeon’s clinical judgment, and unstable group (score of 9 to 14) in which the fusion is needed. Final step of study concluded that this JIS had a high validity and reliability. Conclusion: The newly developed JIS was a valid and reliable scoring system that could help to identify the presence of instability in LSS and can be used as a guideline to decide whether spinal fusion will be needed. © The Author(s) 2024. |
